REQUEST FOR QUOTATION

The Municipality of Pagbilao, through its Bids and Awards Committee (BAC), will undertake Negotiated Procurement (Small Value Procurement) for Documentation/Video in accordance with Section 53.9 of the Implementing Rules and Regulations of the Republic Act No. 9184.

Name of the Project : Pista ng Kulturang Pagbilaoin 2018 - Cultural Activities

Location : Pagbilao, Quezon

Brief Description : Documentation/Video

Approved Budget : P 60, 000.00

Contract Duration : Sixty (60) days upon receipt of Notice to Proceed

A complete set of quotation documents may be obtained by interested proponents on July 19 to 23, 2018 from the BAC Secretariat at the Office of the Municipal Administrator, Pagbilao, and Quezon. Moreover, eligibility documents to be submitted are (i) photocopy of proponent-s DTI or Securities and Exchange Commission Registration Certificate, as the case maybe; (ii) Mayor-s Permit; (iii) BIR Registration; (iv) PhilGEPS Certificate or registration and membership; (v)Tax Clearance; (vi) Income Return Tax; (vii) Audited Financial Statement; and (viii) Omnibus Sworn Statement.

Submission of eligibility and quotation documents is on or before July 23, 2018, 9:15 a.m. at Conference Room, New Administration Building while opening of said documents will follow thereafter at the same address indicated above. Late submission shall not be accepted.

The forms, which are part of the quotation documents, shall be the same forms to be submitted by the proponents for their compliance with the schedule of requirements, technical specifications, and financial proposal submission sheet. All pages of the proposals shall be initialed by the person/s signing the Bid. Non-compliance therewith shall be a ground for disqualification. The -œNo Contract Rule- shall apply. Bidders are not allowed to call or talk to any member of the BAC, TWG, or Secretariat right after the opening of the eligibility and quotation documents.

The proponent determined to have the Lowest Calculated Bid shall be required to furnish the BAC the Certificate of PhilGEPS Registration within three (3) calendar days from receipt of notice thereof.
