PACKAGE 1 PACKAGE 2 PACKAGE 3 PACKAGE 4 1. The subject of the order is the delivery of various medicines according to 4 packages to the headquarters of the Ordering Party (Kraków, os. Złota Jesieni 1), in accordance with the description and requirements contained in Annex 1 to these specifications. 2.For medicines cytotoxic/monoclonal antibodies The Ordering Party requires the data required in the SWZ to be attached to the offer, in particular Annex No. 1 to the SWZ, regarding the density of solutions or the mass of excipients. 3.Expiration date/warranty period of the item the order cannot be shorter than 12 months from the date of delivery, and for food products - shorter than 12 months, but not shorter than 6 months. Deliveries of products with a shorter expiry date may be permitted in exceptional situations and In each case, consent must be given by an authorized representative of the Ordering Party with confirmation via e-mail. 4. In drug and chemotherapy programs, only drugs included in the National Health Fund's reimbursement lists are allowed (the price of the drug cannot exceed the limit financing determined by the National Health Fund). 5. By including the form e.g. "tablet" in the Specification, the Ordering Party means all types of tablets; "cap." means all types of capsules etc.; in other cases, the exact form is specified: e.g. "table. “enteric-resistant” means that only the enteric-resistant tablet is permitted. 6. A medicinal product constituting part of the subject of the order should be understood as medicinal products within the meaning of the Pharmaceutical Law...
