The subject of the order is the delivery of various medicines according to 10 packages to the headquarters of the Ordering Party (Kraków, os. Złota Jesieni 1), in accordance with the description and requirements contained in Annex 1 to these specifications. 2. For cytotoxic drugs/monoclonal antibodies, the Ordering Party requires the data required in the SWZ to be attached to the offer, in particular Annex No. 1 to the SWZ, regarding the density of solutions or the mass of the substance auxiliary. 3.Termin ważności / okres gwarancji przedmiotu zamówienia nie może być krótszy niż 12 miesięcy od daty dostarczenia, a dla produktów do żywienia - krótszy niż 12 miesięcy, jednak nie krótszy niż 6 miesięcy. Delivery of products with a shorter deadline validity may be allowed in exceptional situations and each time consent to them must be expressed by an authorized representative of the Ordering Party with confirmation via e-mail. 4. Only drugs included in the lists are allowed in drug and chemotherapy programs NFZ reimbursement (the price of the drug cannot exceed the financing limit set by the NFZ). 5.
