The subject of the order is the delivery of a stationary X-ray machine and various medical devices for the District Hospital in Radomsko

2. The subject of the order is divided into tasks/packages from 1 to 6:

- Package No. 1 - Stationary X-ray machine with ceiling suspension, 1 piece;

- Package No. 2 - Cardiomonitor with transport module - 2 pcs;

- Package No. 3 - Transport trolley - 10 pcs;

- Package No. 4 - Anesthesia trolley with equipment - 2 pcs;

- Package No. 5 - Medical incubator - 1 pc;

- Package No. 6 - Echocardiograph 1 pc;

3. Detailed Description of the Subject of the OrderAppendix No. 1 to the SWZ assigned to a particular package, i.e. Appendix No. 1/1 to the SWZ, Appendix No. 1/2 to the SWZ, Appendix No. 1/3 to the SWZ, Appendix No. 1/4 to the SWZ, Appendix No. 1/5 to the SWZ, Annex No. 1/6 to SWZ

4. All requirements specified in the documents indicated above constitute requirements, among others:minimal, and their fulfillment is obligatory. Failure to meet the above-mentioned minimum requirements will result in rejection of the offer as inconsistent with the terms of the contract pursuant to Art. 226 section 1 point 5 of the Public Procurement Law

5. The offered medical products must be approved on the territory of Poland, in accordance with the applicable regulationsapplicable legal regulations and have documents confirming that the appropriate procedure for assessing compliance with the requirements specified in the Medical Devices Act, Journal of Laws, has been carried out. 2023 item 974 and be marked with the CE mark.
