Delivery of sodium hypochlorite - sodium chlorate I in the amount of 595.2 tons 1. The subject of the order is the successive delivery of sodium hypochlorite - sodium chlorate I in the amount of 595.2 tons (hereinafter referred to as the "Product") used to disinfect water intended for human consumption, the Contractor's transport to the Goczalkowice Water Treatment Plant street Jeziorna 5, 43-230 Goczalkowice. 2. A detailed description of the subject of the order (OPZ) is included in Annex No. 5 to the SWZ. 3. Detailed conditions for the execution of the order are set out in Annex No. 6 to the SWZ. 4. The means of evidence in question: 4.1. In order to demonstrate that the offered Product complies with the specified requirements in Annex No. 5 to the SWZ - OPZ, the Ordering Party requests that the Contractor submit the following evidence along with the offer: 1) current Certificate Hygienic issued by the National Institute of Public Health - National Institute of Hygiene, authorizing the Product for use in water intended for human consumption; 2) current Permission to trade a biocidal product - sodium hypochlorite - chlorate sodium I issued on the basis of the Act of October 9, 2015 on biocidal products (consolidated text: Journal of Laws of 2021, item 24); 3) Technical Specification of the offered Product. Note: The Technical Specification should confirm compliance with all requirements specified in point 1 of Annex 5 to the SWZ - i.e. in the Product Characteristics.
