Delivery of reagents for biochemical tests with rental of the analyzer for a period of 48 months
1. The subject of the order is: _ supply of reagents for immunochemical tests with the rental of the analyzer for a period of 48 months.
2. A detailed description of the subject of the order includes the price form and specifications technical specifications of the analyzer constituting Annexes No. 1A and No. 4 to the SWZ.
1.1. In order to confirm the compliance of the offered deliveries, the Ordering Party requests the submission of the following evidence together with the offer:
1) 1) Declaration that the offered product range has been approved and introduced to trade in accordance with the provisions of the Act of April 7, 2022 on medical devices (Journal of Laws of 2022, item 974) and has current declarations of conformity and certificates of the notified body and will be made available at any request of the ordering party after signing the contract - the content of the declaration entered in the offer form constituting Appendix No. 1 to the SWZ
2) Analyzer catalog card confirming the parameters indicated in Annex 4 to the SWZ
3) Leaflets for the offered reagents
4) Analyzer declaration of conformity a document issued by the product manufacturer or his authorized representative.
5) Cards manufacturer's catalog and/or instructions for use of the analyzer confirming the assessed parameters (offer evaluation criteria: K2, K3, K4, K5 described in point XV of the SWZ)
