package 1 - basic energy package 2 - energy expanded package 3 - modified energy The subject of the order is the supply of pacemakers for the District Hospital in Chrzanów - 3 packages. The contractor is obliged to do so within 5 days from the date of conclusion of the contract to provide a complete set of products for the so-called "equipment bank" for a hospital pharmacy. The Contractor will agree on the assortment of the "equipment bank" with the Ordering Party's operator. The offered item must be admitted to trading in Poland, in accordance with the Act on medical devices. Offer documents to be submitted: a) Offer form (Appendix 1), b) Detailed cost estimate/priced package(s) (Appendix 2) c) Completed AIT (Technical Information Sheet - Annex 3) - the information contained therein will be assessed (one of the criteria for evaluating offers is technical assessment) a) Single European Procurement Document - in the event of a joint application for a contract by Contractors, a declaration of non-exclusion is submitted by each of the Contractors (Appendix 4), b)Declaration Contractor (Annex 5) that he is not subject to exclusion on the basis of: - art. 7 section 1 of the Act of April 13, 2022. on special solutions for counteracting support for aggression against Ukraine and for protecting national security, - art. 5k Regulation 833/2014 as amended by Regulation 2022/576, as amended. c) Power of attorney authorizing to submit an offer, provided that the offer is submitted by an attorney; d) Power of attorney for...
