Tender Details

1. The subject of the order is the supply of an optical biometer for the Department of Ophthalmology and a surgical drive for the Operating Theater of the Department of Maxillofacial Surgery, University Clinical Hospital. Fryderyk Chopin in Rzeszów, including: Biometr optical for the Ophthalmology Clinic.2. A detailed description of the subject of the order and the requirements are included in the Description of the subject of the order, a list of parameters required for Part No. 1 (Appendix No. 1 to the SWZ) and in the Price and offer form for Part No. 1 (Appendix No. 2 to SWZ), which are an integral part of SWZ.3. The scope of the order includes the supply of the following items: Optical biometer for the Ophthalmology Clinic, 1 piece, described in Annexes No. 1 and 2 to SWZ.4. The Ordering Party does not allow the submission of equivalent offers.5. DEADLINE PERFORMANCE OF THE ORDER: Delivery of the subject of the order to the place indicated by the Ordering Party, installation, commissioning of the delivered devices, staff training and commissioning within 8 weeks from the date of conclusion of the contract.
