The subject of the contract is:
Delivery of non -woven or foil products
Task 1- hygienic apron for the patient
Task 2- surgical cap on an elastic band
Task 3- foil apron
The Employer allows for partial offers.
Parts of the order are individual tasks specified above, i.e. tasks from 1 to 3.
The contracting authority informs that the Contractor may submit an offer to a maximum of 3 tasks.
(without limiting the number of tasks per contractor).
A detailed description of the subject of the contract is included in Annex 2B.
3.3. The contracting authority is related to the subject of the contract:
1) the offered medical product must have approvals, certificates and be registered in accordance with
with laws requiring the possession of approvals, certificates and registration,
2) the offered medical product must be placed on the market and use in accordance with
with the requirements of the Act of 7 April 2022 on medical devices (Journal of Laws of 2024, item 1620, as amended d.),
3) the offered medical product must correspond to the standards or technical specifications in force for this product;
In the event that the offered medical product did not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products. He is obliged to perform at his own expense and on his own all the activities necessary to complete all the requirements of law related to the introduction of the product on the market and use in the Republic of Poland.
