Tender Details

1. The subject of this order is the supply of medical imaging devices divided into parts of the order, called Packages: Package 1: Delivery of an ultrasound machine with equipment, Package 2: Delivery of an ultrasound device for inserting intravenous lines, Package 3: Delivery mobile ultrasound device. 2. A detailed description of the subject of the order, detailing the number, type and requirements, was specified in the Forms of required technical conditions, which constitute, respectively, for Package 1 - 3, Annexes No. 2.1 - 2.3 to SWZ. 3. The Contractor's obligations will include the implementation of the subject of the order in accordance with the provisions of the Draft Agreement, constituting Annex No. 6 to the SWZ. 4.The devices will be delivered by: Package 1: (1M12) Department of Normal and Clinical Anatomy, ul. Chałubińskiego 5, 02-004 Warsaw, Package 2: (1MC3) Children's Clinical Hospital UCK WUM, Department of Pediatric Anesthesiology and Intensive Care, ul. Żwirki i Wigury 63a, 02-091 Warsaw, Package 3: (NZJ) Department of Nursing Development and Social Sciences Medical, Faculty of Health Sciences, ul. Ciołka 27, 01-445 Warsaw. 5.
