Tender Details

1. The subject of the order is the supply of medical equipment, specified in the Parts, for the needs of the University Clinical Hospital. Fryderyk Chopin in Rzeszów. 2. Detailed description of the items of the order, estimated quantity and requirements present: Description of the items of the contract - List of parameters required for Parts No. 1÷9 (Appendices No. 1 to the SWZ) and Price and offer forms for Parts No. 1÷9 (Appendices No. 2 to the SWZ), which are an integral part of the SWZ. 3. The scope of the order includes delivery of the items of the order in the quantities specified in the Price and Offer Forms for Parts No. 1÷9 (Appendices No. 2 to the SWZ) constituting an integral part of the SWZ, for the period: - in Parts No. 1 to No. 3 and No. 5 to No. 9 - 6 months from the date concluding a contract, - in Part No. 4 - up to 7 weeks from the date of conclusion of the contract. 4. The Ordering Party does not allow the submission of equivalent offers in Part No. 1 and in Parts No. 3 to No. 9. 5. The Ordering Party allows equivalent solutions in Part No. 2 as described in the SWZ. 6. ORDER COMPLETION DEADLINE: 1). Delivery of the items of the order in Parts No. 1 to No. 3 and Parts No. 5 to No. 9 - for 6 months from the date of conclusion of the contract. 2). Delivery of the subject of the order in Part No. 4 - up to 7 weeks from the date of conclusion contracts. 3).
