The subject of the contract is:

Delivery of medical devices for electrophysiological examinations

Task 1 - renting a 3D system enabling RF and PFA ablation together with the supply of disposable equipment

The contracting authority does not allow partial offers.

A detailed description of the subject of the contract is included in attachments 2B and 4 to SWZ.

3.3. The contracting authority is related to the subject of the contract:

1. The medical product has been offered must have approvals, certificates and be registered in accordance with the law requiring the possession of approvals, certificates and registration,

2. The offered medical product must be placed on the market and use in accordance with

with the requirements of the Act of 7 April 2022 on medical devices (Journal of Laws of 2024, item 1620, as amended),

3. The medical product has been able to correspond to the standards or technical specifications in force for this product;

In the event that the offered medical product did not require Applications to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products The Contractor is obliged to perform at his own expense and on his own all the activities necessary to complete all the requirements of the law related to the introduction of the product on the market and use in the Republic of Poland. The Contractor is obliged to perform such activities within the time limits arising from applicable regulations and provide the Employer with relevant documents confirming the performance of such activities on these dates.
