Tender Details

1. The subject of the order is the sale and successive delivery upon request of the Ordering Party over a period of 24 months of medical and technical gases, along with the lease of packaging intended for their storage (lease of packaging applies to Task No. 1), divided for tasks: Task No. 1 Medical and technical gases. Task No. 2 Hel. 2. A detailed description of the subject of the order is presented in Annex No. 2 to the SIWZ. The assortment and price form and the OPZ form constituting Annex No. 10 to the SWZ. 1. The contractor has the right to submit only one offer in the form of an electronic document. 2. The content of the offer must correspond to the content of the SWZ. 3. The offer consists of: 1) an offer form completed and signed electronically by persons authorized to represent the Contractor (the template is attached as Annex No. 1 to SWZ) The form does not need to be completed pursuant to Art. 128 of the Public Procurement Law; 2) completed and signed electronically by persons authorized to represent the Contractor. Assortment and price form - (the template is attached as Annex No. 2 to the SWZ) The form is part of the offer and cannot be supplemented pursuant to Art. 128 of the Public Procurement Law; 3) a declaration of not being subject to exclusion and meeting the conditions for participation in the procedure, submitted on the Single European Procurement Document form, abbreviated as "JEDZ" (template constitutes Annex No. 3 to the SWZ).
