Part No. 1 - Insulin pump for children over 6 years of age and adults Part No. 2 - Insulin pump for children under 6 years of age Part No. 3 - Insulin micropump for children over 6 years of age and adults The subject of the order is the supply of pumps insulin. A detailed description and method of order execution are included in the assortment and price table, Annex No. 2 to the SWZ, a list of required parameters, Annex No. 3 (description of the subject of the order) and the contract template, Annex No. 5 to the SWZ. The Ordering Party declares that being data controller and taking into account the provisions of the Act of May 10, 2018 on the protection of personal data (hereinafter referred to as GDPR), it has implemented security measures adequate to the risks it has identified and guaranteeing proper data protection personal data transferred as part of the proceedings. Detailed information clause from Art. 13 GDPR is included in the SWZ. The Ordering Party envisages the use of the so-called reverse procedure referred to in Art. 139 section 1 of the Public Procurement Law.
