Task No. 1 - delivery of a belt cover machine Task no. 2 - delivery of RR Holter devices - 3 pieces Task No. 3 - delivery of ECG Holter devices - 2 pieces; 1. The subject of the order is the delivery, installation and commissioning of a belt cover machine, 3 pieces RR Holter devices and 2 pieces of ECG Holter devices, hereinafter referred to as "equipment". 2. The description of the subject of the order was included in the "Required parameters" form (Annex 2 to SWZ, hereinafter also referred to as "FPW"), this form should absolutely be submitted together with the offer form (Appendix 1 to SWZ, hereinafter also referred to as "FO"). 3. The Ordering Party requires that the offered equipment (does not apply to computer sets offered in tasks 2 and 3) constitute a medical device - within the meaning of the Act of April 7 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620, as amended, hereinafter referred to as "UoWM"), introduced into circulation and use in the territory of the Republic of Poland in accordance with applicable regulations. 4. The Ordering Party requires that the Contractor, when implementing the subject of the contract, acts in accordance with the "do no serious harm" (DNSH) principle, as set out in Art. 17 of Regulation (EU) 2020/852, including in particular: • ensured that the devices meet energy efficiency requirements, • ensured possession of the required environmental certificates, •assured minimizing emissions and environmental impacts, • collected and managed waste generated during delivery, installation or servicing in accordance with the waste management hierarchy (prevention...
