Tender Details

1. The subject of the order is "Supply of implants, prostheses, materials for the treatment of infections, bone replacement materials and medical products to SPZZOZ in Wyszków." 2. The description of the subject of the order is included in Annex No. 2 - Detailed price offer, and Annex No. 5 - Draft provisions of the contract 3. Payment deadline for the execution of the subject of the order: 60 days from the date of delivery of a correctly issued invoice to the Ordering Party. 4.The delivered goods must meet the quality conditions specified in specifications of the order conditions and in the Contractor's tender offer and must have appropriate certificates, approvals and approvals for marketing and use in the health service in accordance with applicable law, including for products classified as medical devices within the meaning of the Act of April 7, 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620) 5. The validity period of the subject of the order must be at least 12 months from the date of delivery. 6. All requirements included in this ToR and Annexes constitute minimum requirements. 7. If any of the product items listed in Annex 2 consists of several elements that will be visible on the invoice after selecting the offer, and have not been indicated in the table above, these items should be priced separately. 8.
