1. The subject of the order is the supply of hematological reagents along with the lease of a basic and backup analyzer to the Laboratory Diagnostics Department - Independent Public Provincial Hospital for Trauma Surgery named after Dr. Janusza Daaba in Piekary Śląskie 41-940 Piekary Śląskie

street Bytomska 62 along with the lease of analyzers described in detail in Annex 2 to the SWZ.

- the scope of the subject of the order includes: delivery and lease of the analyzer, described in detail in Annexes 2 and 6 to the SWZ.

- detailed description of the subject of the order, description of the ordering party's requirements in terms of implementation and acceptance are determined by:

• description of the subject of the order - Annex No. 2 to the SWZ,

• draft provisions of the contract - Annex No. 6 to the SWZ.

All the requirements specified in the documents indicated above are minimum requirements and their fulfillment is obligatory.

Failure to meet the above-mentioned requirements minimum requirements will result in rejection of the offer as inconsistent with the terms of the contract pursuant to Art. 226 section 1 point 5 of the Public Procurement Law.

For the avoidance of doubt, Annex No. 2 to the SWZ includes 10 (ten) detailed annexes, marked as Annexes No. 1, 2, 3, 4, 5 and Annexes No. 1a, 2a, 3a, 4a, 5a to Annex No. 2 to the SWZ.
