Tender Details

1. The subject of the tender is the selection of a contractor who will provide the ordering party with supplies consisting of successive deliveries by the contractor to the District Hospital. Dr. Tytus Chaubiski in Zakopane disinfectants according to the minimum list technical, quality, functional and intended use parameters presented in point 5.10 of the SWZ 17 product items. 2. The Ordering Party stipulates that the delivered subject of the order must have a leaflet of the preparation and manufacturer's statements confirming properties and use of the preparation indicated in the description of the subject of the order, tests confirming the effectiveness of the product indicated in the description of the subject of the order, CE certificate and: 1) in the case of medicinal products, permission to trade in the medicinal product and characteristics of the medicinal product;2) in the case of biomicrobial products, authorization to trade in the biomicrobial product and safety data sheet;3) in the case of medical devices, declaration of compliance with the directive on medical devices and safety data sheets and a statement Contractors that the offered medical devices are in the process of recertification in accordance with the MDR Regulation; 4) in the case of cosmetics, notification...
