1. The subject of the order is: SUPPLY OF DISINFECTION AGENTS for the Provincial Specialist Hospital. Blessed Virgin Mary in Częstochowa. 2. The description of the subject of the order was included in the assortment and price forms, constituting Appendix No. 2 to the SWZ. Other conditions regarding the execution of the order are specified in Annex No. 6 to the SWZ, which constitutes the draft provisions of the contract. 3. The Ordering Party allows the submission of partial offers. A partial offer means individual parts determined in Annex No. 2 to the SWZ. The contractor may submit an offer for any parts or for the entire subject of the contract. 4.
