The subject of this procedure is the supply of computer and videoconferencing equipment for the Medical University of Lodz, including: Package 1 - NAS disk array with disks - 2 pcs; Package 2 - All in One - 60 pcs; Package 3 - device multifunctional - colorful - 1 pc; Package 4 - multifunction device - color - 1 pc; Package 5 - notebook - 5 pcs; Package 6 - videoconferencing bridge - 3 pcs. A detailed description of the subject of the order - quality parameters is attached as Annex No. 2 to SWZ and auxiliary appendix no. 9 to SWZ. Package 1 - NAS disk array with disks - 2 pcs. Before awarding the contract, the ordering party will call on the contractor whose offer was rated the highest to submit within a specified period, not shorter than 10 days from the request, valid as of the date of submission of the following subjective evidence confirming the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion: 1) In order to confirm the lack of grounds for excluding the contractor from participation in procedure, the contracting authority requests the following subjective means of evidence: a) information from the National Criminal Register to the extent specified in Art. 108 section 1 point 1 and 2 of the Public Procurement Law issued no earlier than 6 months before its submission; b)information from National Criminal Register to the extent specified in Art. 108 section 1 point 4 of the Public Procurement Act regarding the prohibition of applying for public procurement as a penal measure - issued no earlier than 6 months before...
