1. The subject of the order is the supply of a computed tomography scanner for Copernicus PL Sp. z o. o. The subject of the order consists of 1 part. 2. The subject of the order consists of the following main activities, i.e.: •delivery of equipment, • preparation of documentation design needed to carry out installation and assembly works and development of a technical design for the calculation of permanent shields along with guidelines for radiological protection, • disassembly and disposal of the equipment currently owned by the Ordering Party tomograph • introducing devices into the CT laboratory room, •installation of equipment in the rooms of the Copernicus PL Sp. Hospital. z o. o. • making all connections of medical equipment to all necessary media needed for proper operation equipment, using existing installation and cable routes and connecting medical equipment to the INFINITT PACS and COMARCH RIS systems owned by the Ordering Party, •starting the equipment and performing procedures to check its correct operation work, •conducting all necessary inspections and obtaining all necessary consents to start working with the use of medical equipment, •conducting training for persons indicated by the Ordering Party • performing website services warranty. 3.
