Delivery of a computed tomography scanner along with disassembly of the currently used one and adaptation of the rooms of the Imaging Diagnostics Department for the needs of the delivered one 1. The subject of the order is the delivery of a computed tomography scanner along with the dismantling of the currently used one adaptation of the rooms of the Diagnostic Imaging Department for the needs of the delivered equipment. 2. The subject of the order is described in detail in Annex No. 1 to the SWZ - Offer form, in Annex No. 2 to the SWZ - List of required quality and technical parameters, in Annex No. 3 to the SWZ - Functional and Utility Program and in Annex No. 5 to the SWZ - Draft Provisions of the Agreement, which provides the quantity of the subject of the order and a description of the subject of the contract. 3. The order is carried out under the contract between Provincial Complex Hospital in the name of Dr. Roman Ostrzycki Konin, and the Greater Poland Voivodeship with the seat of the Marshal's Office of the Greater Poland Voivodeship in Poznań, contract number: DZ-IV-152/W/2025 for: Replacement of a computed tomography scanner in the Department Diagnostic Imaging along with adaptation of rooms. 4. Before preparing and submitting the offer, the contractor is recommended to visit the site of the future place of execution of the subject of the order. The on-site visit will enable Contractors to obtain information useful for preparing an offer, including getting to know the technical condition of the existing facility.
