Tender Details

1. The subject of the order is the delivery, assembly and commissioning of an operating table and an operating table with accessories, hereinafter referred to as "equipment". 2. The description of the subject of the order was included in the "Required parameters" form (Appendix 2 to the SWZ, hereinafter also referred to as as "FPW"), this form must be submitted together with the offer form (Appendix 1 to SWZ, hereinafter also referred to as "FO"). 3. The Ordering Party requires that the offered equipment from task No. 1 and No. 2 constitute a medical device - within the meaning of the Act of 7 April 2022 on medical devices (consolidated text: Journal of Laws of 2024, item 1620, as amended, hereinafter referred to as "UoWM"), introduced into circulation and use in the territory of the Republic of Poland in accordance with applicable regulations. 4. The Ordering Party requires the Contractor in relation to each Task when implementing the subject of the contract, he acted in accordance with the "do no serious harm" (DNSH) principle, specified in Art. 17 of Regulation (EU) 2020/852, including in particular: • ensured that the devices meet energy efficiency requirements, •provided possession required environmental certificates, • ensured minimization of emissions and environmental impact, • collected and managed waste generated during delivery, installation or servicing in accordance with the waste management hierarchy (prevention, recycling use, recycling, recovery, disposal), •transport and installation of devices were carried out in a way that minimized the impact on the environment. 5.
