The subject of the order is the DELIVERY AND IMPLEMENTATION OF UPDATES OF THE RIS/PACS FORMEDICUS SYSTEM THROUGH AUTHOR'S SUPERVISION AND 3 YEARS OF NBD SUPPORT FOR SYSTEMS: Formedicus PACS Panel Formedicus RIS LDO system (2x) HL7 module with existing integration with Asseco's HIS AMMS system The subject of the order was divided into stages (scopes): Stage I: Startup analysis and update to the latest version (eligible cost) Stage IIa: Author's supervision and NBD support for three years (eligible cost) Stage IIb: Service activities (non-eligible cost) 2. The subject of the order has not been divided into parts. 3. Detailed description of the subject of the order can be found in the documentation attached to the SWZ: 1) Description of the subject of the order (OPZ) constituting Annex No. 3 to the SWZ, 2) In the draft provisions of the contract constituting Annex No. 7 to the SWZ.
