Tender Details

1. The subject of the order is the successive deliveries of pharmaceuticals as part of drug programs to the Complex of Health Care Facilities in Cieszyn. The order consists of 43 parts, each of which constitutes a separate subject of the order.

2. Full description of the subject of the order divided into partstogether with the expected demand are included in Annex No. 2 to the SWZ - Price Form.

3. Deliveries will be made successively during the term of the contract based on written orders placed by the Ordering Party's Hospital Pharmacy.

4.The conditions to be met by deliveries are included in pointIII. 3) SWZ.

5. The Ordering Party informs that it is currently implementing and will be implementing during the duration of the order contracts with the National Health Fund under drug programs, therefore it requires the offer of pharmaceuticals included in the List of reimbursed drugs, foodstuffs andspecial nutritional purposes and medical products announced by the Minister of Health, at prices no higher than the official price and within the financing limit.
