Tender Details

The subject of the order is: supplies of medical materials for SPZOZ WSS No. 3 in Rybnik. A detailed description of the subject of the order is included in the annexes: 1) Price form (separately for each package) - Annex No. 1.1. to SWZ, 2)Proposed provisions agreements that will be included in the content of the agreement - Annex No. 2 to the SWZ. The rules specified in the contract also indicate for individual packages, among others: obligations regarding the method and deadlines for delivery, including: lending (applies to package no 29). The order is divided into 29 packages (parts). Agreements regarding the resolved packages will be concluded after the completion of these proceedings for a period of 24 months. The Ordering Party informs that in accordance with Art. 139 of the Public Procurement Law, it will first examine i evaluation of the offers, and then the subjective qualification of the Contractor whose offer was rated highest in a given package, in terms of the lack of grounds for exclusion and meeting the conditions for participation in the proceedings. Fulfilling the obligation arising from Art. 24 section 6 of the Act of June 14, 2024 on the protection of whistleblowers, the contracting authority informs that the procedure for internal reporting of information about violations of law and taking follow-up actions.
