1. The Dupax District Hospital (DDH) had requested for the Supply, Delivery & Installation of Cautery Machine intends to apply the sum of Six Hundred Thousand Pesos (₱600, 000.00) being the Approved Budget for the Contract (ABC) to payments under the contract for charge to (20% DF, 1-07-05-110) to cover eligible payments for which this Request of Quotation is issued.
