Due to two (2) failed public biddings for Circular Proposal No. 2025-84 for the Supply and Delivery of a Passenger Van c/o BFP, may we request your good company to submit the attached Request for Quotation No: CP 2025-84 Negotiated Procurement together with the following documents.

2) Complete brands and Technical Brochures of Offer
3) Certificate of Authorized Distributorship/Dealership and service provider of the vehicle offered
4) Certification as to the availability of spare parts within five (5) years
5) Valid PhilGEPS Certificate of Registration (Platinum Membership) in accordance with Section 20.2.9.1 of the IRR of RA No. 12009
6) Original Omnibus Sworn Statement and if applicable, Original Notarized Secretary-s Certificate in case of a corporation, partnership, or cooperative.
