CPV Description: Waste-tip management services. The service of sorting waste and pressing raw materials on the premises of the Plant. Plant / Installation Bełchatow, ul. Industryowa 14 i 16, 97-400 Bełchat-w, POLAND.

The subject of the order is a service consisting of sorting out mixed packaging waste and pressing sorted raw materials / mixed packaging waste at the Bełchat-w Plant / Installation.

The detailed description of the order is given in item 2 of the ToR.

The ordering party does not place any specific requirements in this regard. Customer orderingand the assessment of compliance with the above condition for participation in the proceedings based on the statement of compliance with the conditions for participation in the procedure - the Employer considers the above condition to be fulfilled if the Contractor submits a policy or other document confirming that the Contractor is insured against civil liability - Within the scope of activities related to the subject orders for the amount of min. 450 000.00 PLN net.

1.In order to demonstrate fulfillment by the Contractor of the conditions referred to in art. 22 ust. 1 of the Act of 29.1.2004. The Law of Public Procurement, the Ordering Party requests the submission of the following documents:

A. The offer form constituting Annex 1 to the SETC-B. Each contractor must attach to the offer an up-to-date statement as per the day of submitting bids in the scope indicated in Annex 3 (EAT) to the SIWZ. Information contained - The statement will constitute preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation.

C. Declaration on employment of employees under a contract of employment, including a list of persons employed.

D. Contractor within 3 days from the date of posting on the website information referred to in art. 86 section 5 of the Public Procurement Law, will provide Zato the declarant, whether he or she does not belong to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. Along with the submission of a statement, the contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure. The pattern of the declaration is attached as Annex 4 to the ToR.

E. Ordering before thighsWith green orders, call the contractor whose offer was the highest rated, to submit in the designated, not shorter than 10 days, the current date as at the date of submission of documents listed in point 7 of the SIWZ.

F. The condition for participation in the proceedings is making a bid bond. The ordering party defines for the whole order a bid bond for PLN 15, 000.00 (say: fifteen thousand zlotys), and the bid bond expires jointly with the offer submission date, ie until 5/10/2018 at 10:00 am The bid bond can be paid in one or more of the following forms: a) money to the account of the Employer at Bank Społdzielczy in Bełchat-w, account number: 24 8965 0008 2001 0006 4288 0001.

Note: Before making a payment in cash, please confirm your account number with the Ordering Party.

B) suretybanking sureties or social-savings credit union guarantees, however, the surety is always a cash guarantee;

c) bank guarantees;

d) insurance guarantees;

e) sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of November 9, 2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2014, item 1804 and 2015, item 978 and 1240).
