The subject of the contract is the rental and service of work clothes for the employees of Master Odpady i Energia Sp. z o.o. as describedcontained in the Specification of Essential Terms of Order.

The subject of the order is the rental and service of work clothing for employees of Master Odpady i Energia Sp. z o.o. in Tychy in the following assortment:
Assortment name number of pieces:
- 204 dungarees,
- men`s trousers 447,
robo sweatshirtcza 651,
- flannel shirt 651,
- winter dungarees 364,
- winter insulated jacket with welt 92,
- 272 winter insulated jacket,
- warning vest 420,
- 609 towels.
Requirements for workwear are detailed described in the ToR.
The Employer does not specify requirements in this respect.
The Employer does not specify requirements in this regard.the following: the offer form:
- EAT, in accordance with Chapter V, point III of the ToR;
- an obligation of a third party, in accordance with Chapter V, point IV, item 3 of the Terms of Reference, if the Contractor intends to rely on technical capabilities to confirm compliance with the conditions for participation in the procedure, or the professional or financial or economic situation of other entities;
- appropriate powers of attorney, declaration according tothe template constituting Annex 2 to the ToR indicating the part of the contract which the Contractor will entrust to subcontractors and subcontractors (if the Contractor foresees the participation of subcontractors and the subcontractors are known);
- sample of clothing.
V. The Employer will call the Contractor whose tender has been highest rated to submit within a specified period, not shorter than 10 days, valida day of submitting declarations or documents confirming that:
1) The Contractor is not subject to exclusion from the proceedings, for the reasons specified in point 1, i.e. the provisions require an entry in the register or records to confirm the absence of grounds for exclusion under Article 24 paragraph 5 points1 of the Act; in the case of a joint offer above a copy is submitted by each of the Contractors submitting a joint bid;
b) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids.
In the case of a joint bid, abovementioned information is submitted by each of the Contractors submitting a joint bid;
c) statementsthe Contractor about the lack of a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or in the case of issuing such a judgment or a document confirming the payment of these debts together with any interest or fines or conclusion binding agreementsand regarding the repayment of these amounts.
In the case of submitting a joint bid, information is provided by each of the Contractors submitting a joint bid;
f) the Contractor`s statement that he has not been subjected to a preventive measure against him being banned from applying for a public contract. information is submitted by each of the Contractors submitting a joint bid;
g) statementsContractors on non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and fees. In the case of submitting a joint bid, the above information is submitted by each of the Contractors submitting a joint bid.
The Contractor meets the conditions for participation in the proceedings referred to in point 2, i.e. :
a) list of services rendered:
- list of services rendered (in accordance withAnnex 3 to the ToR) and in the case of periodic or continuous services also performed, within the last 3 years before the deadline for submission of tenders, and if the period of business activity is shorter in this period, together with their value, subject, dates of performance and entities, for whom the services were performed, and an attachment of evidence specifying whether these services werehave been performed or are being duly performed, the evidence referred to are references or other documents issued by the entity for which the services were rendered, and in the case of periodic or continuous services are performed, and if the Contractor does not, for a justified reason, of an objective nature is able to obtain these documents the Contractor`s statement; for benefits ofreference or continuous references or other documents confirming their proper performance should be issued not earlier than three months before the deadline for submission of tenders;
- list of persons Annex 4;
- price list of assortments in accordance with the provisions of these ToR;
- declaration on belonging or not to the same capital group.
