The subject of the order is the successive provision of laundry services for the Wielospecialist Wojewodzkiego Hospital in Gorz-w Wlkp. Społka with limited liability. A detailed description of the subject of the order and the estimated quantity-included in Appendix No. 2 to SIWZ (price attachment) and Appendix No. 9 to SIWZ (model contract), which are an integral part of the Specification of Essential Terms of Contracts.
Main Site: Hospital at ul. Jana Dekerta 1 and at Walczaka 42 in Gorz-w Wlkp.
The scope of the subject matter of the order includes, in particular:
1. transport of dirty hospital underwear from the point of connection and receipt from the Ordering Party (in the hospital prz ul. Dekerta 1 and at ul. Walczak 42) for the Contractor's laundry.
2. receiving and registering laundry assortment,
3. disinfection,
4. stain removal
5. disinfecting wash,
6. drying -7. padding
8. ironing of linen, bedding and clothing,
9. folding.
10. sorting,
11. packaging,
12. small tailoring services repair of the damaged assortment,
13. transport and unloading of a clean assortmentfrom the contractor's laundry to the Ordering Party's pick-and-collect points (at the Hospital at 1 Dekerta Street and at organizational departments / cells at 42 Walczaka Street),
14. issue to the Ordering Party after the completion of the service sorted and packed individual assortments with a list containing a list of transferred assortments.
The range of the assortment included in the hospital underwear is in particularspears
1. white bed linen (duvet covers, pillowcases, sheets, sleepers),
2. neonatal underwear,
3. baby underwear, children's underwear, diapers, baby rugs,
4. operating underwear and surgical clothing,
5. Protective / fashion clothing (including sweatshirts, pants, skirts, aprons),
6. hospital underwear (pajamas, shirts, bathrobes, etc.),
7. blankets, quilts,
8. pillows (including feathers),
9. mattresses, mattress covers,
10. clothing, underwear, towels for patients (including from the Care and Treatment Department, from the Hospital Emergency Department),
11. work clothes,
12. other (eg curtains, towels, mops, curtains, bags, napkins, treads, screens, etc.),
13. other hospital bed not included in item 1-12
1. Contractors, who are not subject to exclusion, may apply for the award of the contractfrom the proceedings for the award of a contract under Article 24 sec. 1 point 12-23 and 24 sec. 5 Public Procurement Laws
2. In order to demonstrate the absence of grounds for exclusion referred to in paragraph 1 above, the Contractor shall be required to submit the following declarations and documents (described in detail in the SIWZ item XI:
2.1). Current statement as at the day of submitting bids, constituting preliminary confirmatione that it is not subject to exclusion and meets the conditions for participation in the proceedings. The Contractor submits the above-mentioned statement in the form of a document from Food
2.2. A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, issued not earlier than 6 months before the deadline for submission of tenders,
2.3 Informafrom the National Criminal Register to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,
2.4. Certificates from the competent head of the tax office confirming that the Contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of tenders, or another dockthe contract confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular he obtained the legally required exemption, deferral or installment payment of overdue payments or suspension of enforcement of the decision of the competent authority in full;
2.5. Certificates of the local organizational unitThe Social Security Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreement with the competent authority on repaymentsh of receivables with possible interest or fines, in particular obtained legally required exemption, postponement or spreading into installments of overdue payments or suspension in full of the implementation of the decision of the competent authority;
2.6. Contractor's statements about the lack of a final court decision or final administrative decision against him being overdue with taxes, fees or ssocial or health insurance or, in the event of issuing such a verdict or decision, documents confirming the payment of these claims together with any interest or fines or entering into a binding agreement on repayment of such claims.
2.7. Contractor's statements about the lack of a ruling against him as a preventive measure for the prohibition of applying for ordersand public services
2.8. Contractor's statements about not paying taxes and local fees, referred to in the Act of 12.1.1991 on taxes and local fees (Journal of Laws of 2016, item 716) - (model Declaration constitutes Appendix No. 8 to TOR) -3. Eat document referred to in paragraph 2.1. above, the Contractor is obliged to submit together with the offer.
4. Ordering party before orderingwill call the Contractor whose offer has been the highest rated, to submit in the current period, not shorter than 10 days, valid as at the date of submission, documents and statements referred to in paragraph 2.2.- 2.8. above.
The ordering party does not set specific requirements in this respect.
1. The Contracting Party informs that pursuant to art. 24aa of the Public Procurement Law, it will first evaluate offers and thenwill examine whether the Contractor, whose offer has been evaluated as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure.
2. If the offer is signed by a non-party person in the registration document, the offer must be accompanied by the original power of attorney to sign the offer or a copy certified by a notary public for compliance with the original. The power of attorney should be submitted together with the offer.
3. The contractors jointly applying for the order shall submit together with the offer a document establishing their appointment to represent them in the procedure or to represent them in the proceedings and to conclude an agreement for the subject matter of the order. Documents confirming the fulfillment of the subject-matter requirements specified by the Ordering Party, submitted together with the offer: Statement about ownsmanufacturer's document, from which it will appear that the offered tags (chips) have tests confirming the lack of magnetic resonance chips and magnetic resonance on chips, guaranteeing the safety of chip use for humans and electronic equipment such as pacemaker, defibrillator, etc. (pattern document is Appendix No. 4 to the ToR).
5. Contractor's statement about por affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Act. The Contractor shall submit the declaration within 3 days of posting on the website information on the opening of offers referred to in art. 86 par. 5 of the Act
6. In order to confirm the fulfillment of the subject-matter requirement, the Contractor will be required to present at the request of the Ordering Partydocuments:
a. a valid certificate or other document issued by a competent State Sanitary Inspector, confirming that the laundry services provided by the Contractor meet the requirements for washing hospital underwear while maintaining a hygienic barrier, including contaminated underwear and underwear from neonatal units; current certificate or other document issued bya competent State Sanitary Inspector confirming that car vehicles used to transport clean and dirty hospital underwear meet the sanitary and epidemiological requirements. the producer's document, from which it will appear that offered tags (chips) have tests confirming the lack of impact of chips on magnetic resonance, as well as magnetic resonance on chips, gwasecurity of the use of chips for people and electronic equipment like a pacemaker, defibrillator, etc.
