The subject of the order is to provide a training service in the field of organizing and conducting 8 training and activation cycles for osat in penitentiary units subordinate to OISW in Szczecin under the project "Raising professional qualifications of prisoners in order to return to the labor market after imprisonment" under the Operational Program Knowledge Education Development 2014-2020, financed from the funds of European Funds.

3 training and activation cycles with courses: Hairdresser, Waiter, Flat surface conservator.
• Hairdresser's course and vocational activation course 1 training cycle;

Description of individual training cycles:
Men's Barber
Number of didactic hours of the training and activation cycle for the participant 83 hours, including:
• activation course 20 h didactic • training course for 63 teaching hours, including: - first aid 6 h - theory course--12 h
- practice course 45 h
The course includes theoretical and practical classes preparing the participant to perform the barber's profession.
Thematic scope of the training (theory and practice)
• principles of first-aid first aid;
• occupational health and safety;
• disinfection, sterilization, maintenance of tools;
• creating a professional workplace;
• professionalsfor customer service: welcoming the client, consultation, diagnosis, proposal;
• review of tools and preparations for treatments;
• structure and properties of the hair;
• analysis of face and head shapes;
• selection of hairstyles to shape the face and facial hair;
• haircut mens hair;
selection of beard styling to face shape;
• beard and mustache haircut;
• hair and beard care.
Waiter
Number of teaching hours-the training and activation cycle for the participant is 106 h, including: • 20 h educational activation course • training course for 86 teaching hours, including: - first aid 6 h - theory course 24 h - practice course 56 h
The course covers theoretical and practical classes preparing the participant to perform the job of a waiter.

The number of teaching hours of the training and activation cycle for the studentthe spike is 92 h, including:
• activation course of 20 teaching hours
• training course for 72 teaching hours, including:
- first aid 6h
- course theory 24h
- practice course 42h
Course includes theoretical and practical classes preparing the participant to perform the profession of a conservator of flat surfaces.

• rules for the provision of the firstpre-medical assessment;

• organizing the workplace;
• preparing tools and means for individual protection;
• chemical agents, their types and applications;
• types and types of floor;
In order to confirm that the Contractor meets the requirements set out in Chapter 5, point 1 lit. and the terms of participation in the proceedings regarding competences or entitlements toThe Employer will request documents confirming the possession of such competences or rights, if the obligation to own them results from separate regulations, in particular: • a concession, permit, license or document confirming that the Contractor is registered in one of the professional or trade registers, carried out in a Member State of the European Unionin which the Contractor has his registered office or place of residence.
In order to confirm that the Contractor meets the requirements set out in Chapter 5, item 1 lit. b of this SIWZ terms of participation in the proceedings regarding the economic or financial situation, the Employer will request the following documents: • documents confirming that the Contractor is insured against civil liability in the scope of conducted-activities related to the subject of the order for a guarantee sum not less than PLN 50, 000.00 (in words: fifty thousand zlotys).
The grounds for exclusion - Pursuant to art. 24 sec. 1 item 12, 13, 14, 15, 16, 17, 18, 19, 20, 21, 22, 23 and art. 24 sec. 5 points 1, 2, 3, 4, 5, 6, 7, 8 (Journal of Laws of 2017, item 1579 with amendments)
List of statements or documents confirming that the conditions for participation inproceedings and no grounds for exclusion
- A completed EAT should contain at least the following information:
• Contractor's statement that there are no exclusion conditions in relation to the Contractor;
• Contractor's statement on meeting the conditions of participation in the proceedings;
Contractor's statement on compliance by him selection criteria to limit the number of Contractors• specifying the public authority or third party responsible for issuing documents confirming the lack of grounds for exclusion and, if applicable, meeting the conditions for participation in the proceedings and selection criteria; - a formal statement from the Contractor, from which it follows that the Contractor will be able to present documents on request and without delayconfirming the lack of grounds for exclusion and fulfillment of the conditions for participation in the selection procedure and selection criteria • information necessary to obtain by the Employer documents confirming directly by means of a database.
In order to confirm the lack of grounds for exclusion of the Contractor referred to in Chapter 6 of this SIWZ The Employer will request: • information from the National Karna Registerego within the scope defined in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the offer submission or requests to participate in the proceedings, • certification of the competent head of the tax office confirming that the Contractor is not in arrears with tax payment, issued not earlier than 3 months before the deadline for submission of offers, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension in full of the decision of the competent authority, confirmation of the appropriate unit of the unit organizacyjna Zakład Ubezpieczeń Społecznych or Kasa Rolniczego Ubezpieczenia Społecznego or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority in the matter-repayment of these receivables together with any interest or fines, in particular obtained legally permissible, postponement or payment of overdue payments or suspension of the execution of the decision of the competent authority in full, - an excerpt from the relevant register or from central records and business information, if separate regulations require an entry in the register or ewiin order to confirm the absence of grounds for exclusion under Article 24 sec. 5 point 1 of the Act. Condition of participation in the proceedings - Contractors whose activity, or the activity of their organisationally separated units that implement the contract, will be eligible for the awarding of contracts, including the social and professional integration of persons who are members of socially marginalized groups, i.e.. persons deprived of their liberty or released from penitentiaries, referred to in the Act of 6.6.1997, Kkw (Journal of Laws of 2017, item 665), having difficulties in integration with the environment. The minimum average employment rate of the above mentioned persons must be at least 30% in the second quarter of 2018, ie (1.4.2018-30.6.2018). In the case of a consortium, each participant must fulfill the conditiony co-operative. - Criterion 1. Offer price - 60%, 2. Average percentage employment rate people - 40%
