Sanitary-veterinary actions included in the program of actions for surveillance, prevention, control and eradication of animal diseases, of those transmissible from animals to humans, animal protection and environmental protection, identification and registrationbreeding of cattle, pigs, sheep, goats and equidae, of the actions provided for in the program of supervision and control in the field of food safety.

Main Site: Within the Baia Mare constituency.

According to the specifications. must be found in the situations provided in art. 164, 165, 167 of Law no. 98/2016. For deshowing the fulfillment of this requirement, all economic operators participating in the procedure will complete DUAE with the information related to their situation.

Declaration regarding the non-compliance with the provisions of art. 59, 60 of Law 98/2016, regarding the persons holding decision-making functions within the contracting authority, as follows:

- IIf in the country of origin or in the country in which the candidate is established, no documents are issued regarding all the situations stipulated by the DUAE, a declaration will be accepted on his own responsibility or if in the respective country there are no legal provisions regarding the declaration on his own responsibility, an authentic statement given in front of a notary, an administrative or judicial authority or ato a professional association that has competence in this regard,

- Documents issued in a language other than Romanian will be presented in the language in which they were issued, accompanied by an authorized translation of these into Romanian. All documents will have, besides the signature, clearly mentioned the full name of the person signing,

- If the offer is submitted by an association, each associationt (including the leader) will present the DUAE. If the person who signs the requested statements / forms is not the legal representative of the tenderer (leader, associate), a proxy for Form 1 will be attached to it. contracting only by the bidder classat the first place after applying the award criterion according to art. 196 paragraph (2) of Law 98/2016. The documents requested by the economic operator ranked first will be uploaded to SEAP, scanned and with the extended electronic signature. The economic operators submitting the offer must prove a form of registration according to the law of the country of residence, to prove that the economic operator isit is legally constituted, as it is not in any of the situations of cancellation of the constitution as well as the fact that it has the professional capacity to align the activities that are the object of the contract. Method of fulfillment: completing DUAE, following that the supporting document, respectively ONRC certificate or for foreign bidders, equivalent document issued in the country of residence, to be presented dat the request of the contracting authority to complete the evaluation of the offers. Registration certificate issued by the Trade Register Office (for P.J.), the certificate of registration in the unique register of veterinary medical offices (for P.F / P.J) completing DUAE.
