Subject of order is the supply of brand new equipment and mats. office buildings up to 35 WOG and and institute in stock. Zamathe believer divided the subject of the order into three lots:
The subject of the order for lot No. 1 is the purchase and delivery of brand new office equipment and devices (hereinafter referred to as the Goods) for the 35 Military Economic Department with headquarters in Rząska at ul. Krakowska 2 to the place of delivery indicated by the Employer.
The Employer has not set detailed requirements in this respect.
Net value in PLNazana in section II.1.5 contains the estimated value of the contract based on the provision of art. 6a of the Public Procurement Law, ie: PLN 40, 650.41.
The Contractor, when preparing the offer, see is to attach to the print the "Offer" (appendix 1 to the ToR suitable for the given part of the order) No. doc. any announcements about a change of announcementa;
b) "Form. price. "(Annex No. 5);
c)" A notice on putting at the disposal of the necessary resources for the period from to "in order to use them for order execution" (Annex No. 2) submitted in the original and signed by the person (s) authorized to represent the entity making the resources available in the footnote use of other sub-resources;

e) doc. regarding the criteria for the evaluation of the tenders referred to in part I item 14 of the ToR;
f) power of attorney to represent all Contractors jointly applying for the contract.
An attorney may be appointed to represent Contractors in proceedings or to represent in proceedings and conclude a contract (if applicable);
g) the original bid security if the bid security is paid inother form than in money.
Contractor to demonstrate the lack of grounds for excl. from the procurement procedure in the circumstances referred to in art. 24 paragraph 1 and art. 24 paragraph 5 of the Public Procurement Law, is obliged to submit at the request of the Employer based on the provisions of Art. 26 section 1 of the Public Procurement Law Act, the following documents:
a) an excerpt from the relevant register or from the central register and information oneconomic viability, if separate provisions require an entry in the register or records to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;
b) a certificate from the competent head of service confirming that the Contractor is not in arrears with taxes, issued not earlier than 3 months before the deadline for submitting bids or applications for admission to the thighsin the proceeding, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full the implementation of the decision authority;
c) relevant certificatesthe local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social or health insurance contributions, issued not earlier than 3 months before the deadline for submitting offers or requests to participate in the procedure, or other documentconfirming that the Contractor has entered into an agreement with the competent authority on the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision of the competent authority;
d) information from the National Criminal Register in the scope specified in art. 24 paragraph. 1 item 13, 14 and 21 of the Act and, regarding conviction for an offense to imprisonment, to the extent specified by the Employer pursuant to Art. 24 paragraph 5 points 5 and 6 of the Act, issued not earlier than 6 months before the expiry of the time limit for the submission of tenders or requests to participate.
