IROP Perinatology Phototherapy Lights

The subject of public procurement (VZ) is a supply of 5 pcs of Phototherapeutic Lamp for the Department of Gynecology and Obstetrics Clinics General University Hospital in Prague (VFN), transport, installation, putting into operation, execution of instructions according to the relevant provision of Act no.-268/2014 Sb. on Medical Devices (Act No. 268/2014 Coll. training of the buyer and handing over of the documents relating to the goods to be supplied, especially the declaration of conformity and instructions for use in the Czech language in printed and electronic form including the guarantee service and further according to the other conditions of the documentation. Subject covered publicly(VZ) is a contractor of 5 Phototherapeutic Laboratory Lamps for the Department of Gynecology and Obstetrics Clinics General Faculty Hospital in Prague (VFN), including accessories, duties, transport, installation, putting into operation, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. . on Medical Devices (Act No. 268/2014 Coll. training the buyer's handler and handing over the documents, -including the warranty service and, in accordance with the other conditions, the supporting documentation. (a) a written record from the trade register, if it is entered in the register, or a copy of another similar to that in which the supplier is registered under a special legal regulation (Section 77 (1) of the Act);-(c) a document attesting to its fitness for distribution and servicing of medical devices, ie a document certifying its fitness for distribution and servicing of medical devices, ie a document proving its competence for the distribution and servicing of medical devices, one of the following options: - About the confirmation of the registration of the registration issued by the SLAK or About the Extract-from the register of SLAK pursuant to Act No. 268/2014 Coll. or the URL to which the registration can be verified

(if a medical device is provided according to Act No. 268/2014 Coll.)

d) a document certifying the manufacturer's authorization to distribute and service the subject VZ
