Handling domestic and foreign business travels of the Office of Technical Inspection ZP-DA-44/19

The subject of the order is to service domestic and foreign business travels of UDT. The Contractor undertakes to the Employer to perform the services ordered:

Main Site: 1. Place of performance of the contract: Urząd Dozoru Technicznego, ul.Szczesliwicka 34, Warsaw, POLAND or other place indicated by the Employer.

The subject of the order is to service domestic and foreign business trips of UDT. The Contractor undertakes to the Employer to perform the services ordered:

a. booking, purchase and delivery of air tickets for international and domestic flights, domestic and international rail transport and ferries, includingcollision-free relation of multi-stage connections and the shortest connections on a given route, including the lowest published tariffs and negotiated rates as well as promotional rates in the classes required by the Employer, as well as in the class of carriers low cost airlines,

b. hotel reservations abroad, organization of transfers, car and bus rentals,

c.brokering visa formalities,

d. cost optimization of Employer`s employees` travel

1. Contractors who can apply for the contract who:

a) are not subject to exclusion pursuant to art. 24 paragraph 1 and art. 24 paragraph 5 of the Public Procurement Law;

b) meet the conditions for participation in proceedings regarding technical or professional ability regarding experience.

2. Documents or statementsvigilance to confirm the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion:

form of the single European order document (single document), i.e. the Contractor`s current statement as at the date of submission of tenders, constituting the preliminary confirmation that the Contractor is not subject to exclusion in the scope of which referred to in art. 24 paragraph 1 and art. 24 paragraph 5 of the Public Procurement Law

3. Other documents and statements:

1) offer form - template Annex 1 to the ToR,

2) commitment of another entity (if applicable) Annex 3 to the ToR,

4. The Contractor whose offer has been assessed as the most favorable to confirm that it is not subject to exclusion and meets the conditions for participation in the procedure, will be invited to submit an act within a time limit not shorter than 10 daysas at the date of submitting statements or documents confirming the circumstances referred to in art. 25 paragraph 1 of the Public Procurement Law:

a) in order to confirm that the Contractor meets the condition of participation in the procedure referred to in the ToR - the Awarding Entity requests:

1) a list of services and evidence specifying whether these deliveries were made properly - in accordance with point III.1.3) announcement,

b) to confirm bratowards the grounds for excluding the Contractor from participation in the procedure, the Employer requests the following documents:

1) information from KRK - par. 5 paragraph 1 of the Regulation on documents (Journal of Laws of 2016, item 126, as amended),

2) a certificate from the competent head of the Tax Office confirming that the Contractor is not in arrears with payment of taxes - par. 5 paragraph 2 of the Regulation,

3) certification of the competent field organizational unit of the ZUS or KRUS or another document confirming that the Contractor is not in arrears with fees social security contributions or healthy - parish 5 paragraph 3 of the Regulation,

4) an excerpt from the relevant register or CEIDG - par. 5 paragraph 4 of the Regulation,

5) the Contractor`s statement that there are no grounds for exclusion - according to the attachment to the ToR.

The Contractor who has its registered office or place of residence outside the territory of the Republic of Poland submits documents fromworthy of parish 7 of the Regulation on documents (Journal of Laws of 2016, item 126, as amended).

In accordance with art. 22a of the Public Procurement Law, the Contractor may rely on technical or professional ability or financial or economic standing of other entities to confirm compliance with the conditions for participation in the procedure, in appropriate situations and in relation to a specific contract or part thereof.and the legal nature of the legal relations which it has with him. The Contractor, which relies on the abilities of other entities, must prove to the Employer that in carrying out the contract, he will have the necessary resources of these entities, in particular, presenting the undertaking of these entities to make available to him the necessary resources for the purposes of the contract, according to Annex 3 to SIW-Z CAKE. The commitment must be made in the original together with the offer.

The contractor is required to pay a deposit of 30, 000 PLN.

The deadline for submitting the bid security expires along with the deadline for submitting bids.

The bid security can be lodged in one or several of the following forms:

a) money - security to be transferred via bank transfer to the Purchaser`s bank account:

The deposit in cash will be effective if it is placed on the Purchaser`s bank account within the deadline for submitting bids;

b) bank sureties or sureties of the Cooperative Cash Register Savings and Credit, except that guaranteeing the cash register is always a monetary guarantee;

c) bank guarantees

d) insurance guarantees;

e) sureties granted by entities referred to in art. 6b paragraph 5 point 2 of the Act of November 9, 2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2007, No. 42, item 275, as amended). Deposit in a form other than in cash submit with the offer or by e-mail to the address: Przetargi@udt.gov.pl, in electronic formhey, bearing a qualified electronic signature by the Guarantor, i.e. the issuer of the guarantee / surety.

Note: the content of the guarantee should result unconditional, irrevocable, upon each written request submitted by the Employer within the period of the offer, the Guarantor`s obligation to pay the Employer the full amount of the bid bond in circumstances specified in art. 46 section 4a of the Public Procurement Law and in art. 46 section. 5 of the Public Procurement Law

The proceedings are conducted pursuant to art. 24aa section 1 of the Public Procurement Law, so-called reverse procedure, which means that the Employer will first evaluate the offers and then examine whether the Contractor whose offer has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure.

The Contractor, within 3 days of posting on the websitethis information from the opening of offers, referred to in art. 86 section 5 of the Public Procurement Law, provides the Employer with a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act. The Contractor may provide, together with the declaration, evidence, documents or information confirming that links with another Contractor do not lead to disruption of thecompetition in the proceedings. The Employer will perform activities resulting from the instructions of Art. 87, art. 89 and art. 90 clause 1 of the Act. The Employer shall perform a subjective analysis in relation to the highest rated Contractor in terms of the grounds for exclusion and fulfillment of the conditions for participation in the procedure.
