Maintenance of a pre-selective vehicle weighing system in motion along national roads administered by GDDKIA Branch in Gdańsk.

Reference Number: O.GD.Z-2.2413. 13.2018

The order consists in performing maintenance, throughout the entire period covered by the contract, of all preselection stationsin the national roads of the Pomeranian voivodship, administered by the General Branch of the Directorate of National Roads and Highways in Gdansk.

Main Site: North Region, province Pomeranian.

1. The order consists in:

1) Stage I

a) Recreation of the pre-selection station in Nicponia on the national road No. 91 at km 75 910L;

b) Repair of the damaged pre-selection station in the town of Ĺšino on the national road No. S6c at km 6 100P;

c) Adaptation of the pre-selection system of vehicle weighing in motion to full functionality, including

• adaptation (assembly and service work) of existing preselection station equipment, including: replacement of all worn-out system components, i.e. weight sensors, sensors for exceeding the permissible height, inductive loops, teletechnical cabinets, devices for recording video, etc.

• integration (preparatory and organizational work) of a preselection system of weighing vehicles in motion within national roads administered by GDDKiA OddziaĹ, in Gda- "sk, including: registration, transfer of data between pre-selection stations and server, ensuring efficient system operation, providing data from all pre-selection stations on a website maintained in Polish.

d)Implementation of software for the system of weighing vehicles in motion and running an Internet application.

2) Stage II

a) Maintenance of all preselection stations of weighing vehicles in motion, located along the national roads in the Pomeranian voivodship (including emergency events), following the protocol completion of works falling within the scope of Stage I.

2. The orderer requires that the subject of the orderThe decision was made not later than 48 months from the date of concluding the Agreement, keeping the intermediate dates specified in the Terms of Reference: -a. The Contracting Authority requires that Stage I orders, ie the pre-selection station be restored in Nicponia on the national road No. 91 in km 75 910 L, overhaul of the pre-selection station in Łosino on the S6c expressway at km 6 100P, adjustment of the pre-installationselective weighing of vehicles on the move to full functionality (adaptation, integration) and execution of software for the vehicle weighing system in motion along with running the web application (completed with the acceptance protocol) was made within 120 days of signing the contract. b) Ordering requires that Stage II orders, i.e. the provision of maintenance services, begin-from the date of completion of Stage I and the Ordering Party signing the final acceptance protocol of Stage I until the end of the contract (not less than 44 months).

1. The Contractor is obliged to submit a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractor: a) is not subject to exclusion, b) meets the conditions for participation in the procedure. AxisThe certificate referred to in point 1 above (in the form of a single European order document prepared in accordance with the standard form specified in the EC Implementing Regulation issued on the basis of Article 59 (2) of Directive 2014/24 / EU, hereinafter referred to as a "single document" or "JEDZ"), the Contractor is obliged to send the Ordering Party in an electronic form bearing the qualifiedin electronic form, in accordance with the principles set out in point 12.3 of the ID-3. In terms of "Part IV. Eligibility criteria" EAT, the Contractor may limit itself to completing the alpha section, in which case the contractor does not complete any of the remaining sections (AD) in Part IV., to demonstrate that there are no grounds for exclusion against them, and-compliance, in the scope in which it refers to their resources, conditions for participation in the proceedings, submit uniform documents also regarding these entities. Contractor who relies on the abilityor the situation of other entities, he must prove to the orderer that while performing the order, he will have the necessary resources of these entities, in particular by presenting the obligation of these entities to give him the necessary resources for the needs of the order.

6. Contractors may jointly apply for a contract. In such a case, the Contractors shall establish the fullhe is trying to represent them in the proceedings for the award or representation in the proceedings and conclusion of the public procurement contract.

7. In the event of a joint application by the Contractors, a single document in the form of an electron. equipped with a qualified electronic signature shall be submitted by each of the Contractors jointly applying for the order. 9. If the Contractor has his registered office or place of residence outside the territory of the Republic of Poland, as well as in the case where the person the document relates to is residing outside the territory of the Republic of Poland, the rules specified in art. in - 7 and 8 above Ordinance.

10. Contractor, within 3 days from the date of posting on the website. information referred to in art.-86 par. 5 uPzp, transfers to the Ordering Party about belonging or not belonging to the same capital group, referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. Along with the submission of the statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure

11. Proceedings for the procurement the Contractor shall be excludedwhich any of the circumstances referred to in art. 24 sec. 1 point 12 23 and art. 24 sec. 5 point 1 4 and point 8 of the Public Procurement Law

12. Customer, acc. from art. 24 aa of the Public Procurement Law, provides for the possibility of first of all assessing the offers, and then of examining whether the Contractor whose tender has been assessed as the most advantageous is not subject to exclusion and meets theunki share in the proceedings.

13. The period of being bound by the offer referred to in point IV.2.6) of the offer is 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires
