The subject of the order is the purchase of tractors with accessories for current road maintenance. A detailed description of the order and the quantities is specified in chapterle III

Main Site: Place of delivery: Poviat Road Administration in Białystok, Zaścianki, Szosa Baranowicka 37, 15-522 Białystok, POLAND

Purchase of the following assortment: agricultural tractor 3 pcs, multifunction boom 3 pcs, flail head 3 pcs, trunk milling machine 1 pc., road shoulder trimmer 1 pc, ditch miter 1 pc, flail mower on pantograph 3 pcs, flail mower 3 pcs., scissor trimmer 1 pc, snow plow 3 pc, sweeper 3 pc

The bid must be secured with a deposit in the amount of: PLN 32, 000.00. 24 paragraph 1 point 12-23 of the Public Procurement Law

The Employer does not provide for exclusion on the basis of the conditions referred to in art. 24 paragraph 5 of the Public Procurement Law

In order to pre-confirm that the Contractor has failed tosubject to exclusion and fulfilling the conditions for participation in the procedure, the Contractor submits to the offer:

1. single European order document / ESPD (hereinafter referred to as "EAT") on or according to Annex 1. The document should be completed in accordance with the instructions on the website www.uzp.gov.pl, tab: Repository of knowledge / Single European order document .

2. There is a possibility of fillingEAT form using the ESPD system available via the following website https://ec.europa.eu/growth/tools-databases/espd/ or https://espd.uzp.gov.pl/. The form pre-prepared by the Employer for the said procedure in electronic form (in .xml format - to be imported on the ESPD website) is available on the Employer`s website.

3.EAT for other entities, if applicable.

The contractor may use in JED the still current information contained in another JEZ submitted in a separate contract award procedure. against them the grounds for exclusion from participation in the progress

List of statements or documents submitted by the Contractor in the proceedings, at the request of the Employer, to confirm the circumstances referred to in art. 25 paragraph 1 point 3 of the Public Procurement Law - applies to the highest rated offer:

8.1. The contractor submits:

8.1.1. Information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the PPL Act, issued not earlier than 6months before

the offer submission deadline;

8.1.2. Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or - in the event of such a judgment or decision being issued - documents confirming the payment of such claims together with anyinterest or fines or the conclusion of a binding agreement on the repayment of these debts;

8.1.3. Contractor`s statement on the lack of a decision against him as a preventive measure prohibiting him from applying for public contracts.belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law, together with other Contractors, who submitted offers in this procedure, Annex 3.

In order to confirm that the offered deliveries meet the requirements specified by the Employer, the Contractor whose offer will be rated the highest will deliver within the time limit set by the Employer ( not krots(less than 10 days):

The exact technical description of the equipment offered in relation to the description given by the Employer in Chapter III - Description of the object of the contract (the description may not contain imprecise wording such as "or", "e.g.", "not less than") The description should clearly show what kind of equipment, about which parameters the Contractor offers and should be identical to the equipment inserted in the form.offer (model, producer) under pain of rejection. The technical description of the offered equipment should be made on / according to Annex No. 2.
