The subject of this part of the contract is the delivery of a new non-scheduled radiological imagingcontrol used for diagnostics - delivery of new multidetector computer tomograph with reconstruction of 128 layers including workstation for physician based on client-server solution for diagnostics and postprocessing and application software equipment of workstation for physician for diagnostics and postprocessing (1 pc device 128 slice). The subject of this part of the contract is delivery24 months, transport to the place of delivery, installation and installation at the place of delivery, execution of the trial and trial operation and putting the equipment into operation, first operator training. ZVO). This fact is manifested in the way mentioned in the FTA. In the case of vendor groupsthe procedure pursuant to Section 37 of the FTA shall be applied.

1. The price must include all costs associated with the implementation of the subject matter, including all related services and fees, includingcommunication in accordance with the subject matter specification. In the event that the applicant applies a single European document (JED), he / she is required to complete it in accordance with the ban on public procurement. Documents and documents temporarily replaced by the JED must show the valid status of the tender at the date of submission of tenders. 5 The submission of the bid is only possible by an authenticated bidder. Authentication can be accomplished using the following methods:

- in the systemJosephine signing up and signing up with an ID card with an electronic chip and security personal code (eID). The system is authenticated by a company that registers the statute of the company using the eID. Authentication is done by the system provider Josephine, on weekdays 8.00-16.00 hrs,

- uploading a qualified electronic signature (e.g.eID) of the company 's statutory user card after registration and login to Josephine. Authentication will be performed by the system provider Josephine, on business days at 8 am to 4 pm, - by placing a power of attorney on the card of the user after registration, which is signed by the statutory electronic signature and the authorized person, or has undergone a guaranteed conversion. Authentication by ProviderJosephine system operator, working day 8.00-16.00 hrs,

- by waiting for the authentication code, which will be sent to the address of the firm's address in the hand of the registered applicant in the form of registered mail. Usually, the deadline for this termination is 3 business days and it is necessary to count with this deadline when bidding. Due to the nature of the subject matter of the order, the inspection of the place of execution (delivery)is recommended for part no. 1 CT instrument

7. Bid security is required for:

Part 2 USG Portable at EUR 1, 200.00

Part 3: USG for operational cost of EUR 2, 300.00

Part 4: USG Gynecological at EUR 1, 100.00

Bidder is required to deposit security for the part of the subject of the job on which it submits the bid. More information on the submission of security is provided in the sources of support. 17
