The subject of the order is the delivery, installation, assembly and commissioning of computer tomographsch along with equipment, training in the use of equipment and design services, adaptation and installation work.

1. The subject of the order is the delivery, installation, assembly and start up of computer tomographs (CT) along with equipment, training in the use of equipment and the performance of the servicedesign services, adaptation and installation works. The subject of the order includes: - District Hospital in Ustrzyki Dolne:

1) Implementation of the design documentation of the central sterilizer and computer tomography laboratory together with the radiological protection project, permanent shields design and the implementation of specialist acceptance and acceptance tests to control CT technical parameters, including the notification and supervision on delivery carried out by the appropriate local sanitary inspection unit. Execution of a construction and executive design, adaptation of the indicated rooms to the CT laboratory's premises to the extent necessary to meet all installation guidelines of the manufacturer and the central sterilization room, RTG apparatus (construction works, including:golnobudowlane adaptation of premises, finishing works, construction of intermediate structures, delivery of carpentry, electrical installations, including, among others: power line for CT equipment, execution of switchboard with signaling and control installation for CT equipment, provision of power to ventilation and air conditioning systems, comprehensive electrical installations, installations sanitarian, including plumbing, heating systems, C.O., ventilation and air conditioning, design of radiological covers and their execution, preparation of the transport route). The Employer places a projection of the rooms covered by the adaptation of Annex 2a to the ToR.

2) transferring the central sterilization equipment (located in the place of the future CT laboratory) to the indicated premiseswith the adaptation of designated rooms to conduct its operations in a manner ensuring its reception by authorized institutions (including WSEZ and UDT).

3) Delivery and assembly of CT along with the equipment described in Annex 2 to the ToR;

4) Obtaining acceptance and final permits related to radiological protection and ionizing radiation and construction regulations;

5) Execution of tacceptance and specialist estimate after installation of CT and delivery of documents necessary to release the device for use and obtaining all permits required by law;

6) Launching the installed CT;

7) Training according to Annex No. 2 to the ToR.

8) Performing service works, periodic inspections, ongoing maintenance in accordance with the manufacturer's recommendations, performing repairs that ensure efficient and safe operation of CT and ensuring the continuity of operation of the X-ray tube in the CT.

9) The Ordering Party reserves the right to credit in the scope of CT with the equipment.

Wojewodzki Szpital im. St. Ojca Pio in Przemyśl

1) adaptation of the existing CT laboratory to the extent necessary to meet all the manufacturer's installation guidelines.

2) Delivery and assembly CTwith the equipment described in Annex 2 to the ToR,

3) Obtaining approvals and final permits related to radiological protection and ionizing radiation and building regulations (if they are required by applicable regulations);

4) Performing acceptance and specialist tests after installation of CT and delivery of documents necessary for the admission of devicesia for use and obtaining all permits required by law;

5) Launching the installed CT;

6) Training in accordance with Appendix 2 to the ToR.

7) Performing service work, periodic inspections, ongoing maintenance in accordance with the manufacturer's recommendations, making repairs to ensure efficient and safe operation of CT and ensuring the continuity of operation of the X-ray tube in CT.

8) Zamthe entrant reserves the option of lending in the scope of a computer tomograph with equipment.

A detailed description of the order No. 2 to SIWZ

The Ordering Party considers that the condition is met if the Contractor shows that he has financial means or creditworthiness for an amount not less than 3, 000, 000, PLN 00 (in words: three million zlotys).

According to art. 24aa paragraph 1 Pzp. Ordering agentIt is possible to assess the offers first, and then to check whether the Contractor whose offer has been evaluated as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings. Contractors, who are not subject to exclusion pursuant to art. 24 sec. 1 point 12-23 of the Public Procurement Law - 2. According to art. 24 sec. 8 mouthsPzp, the Contractor who is subject to exclusion pursuant to art. 24 sec. 1 item 13 and 14 and 16-20 may provide evidence that the measures taken by him are sufficient to demonstrate its reliability, in particular to prove compensation for damage caused by an offense or fiscal offense, financial compensation for damage suffered or compensation, exhaustive explanationfactual cooperation and cooperation with law enforcement agencies and taking specific technical, organizational and personnel resources that are appropriate to prevent further offenses or fiscal offenses or improper conduct of the contractor. The provision of the first sentence shall not apply if the contractor, being a collective entity, has been convicted by a final court judgmentfrom the application for the order and the period of validity of the prohibition specified in that judgment has not expired.

3. The Contractor is not subject to exclusion pursuant to art. 24 sec. 1 point 13 and 14 and 16-20 if the Awarding Entity, taking into account the weight and special circumstances of the contractor's deed, considers that the evidence provided by the Contractor is sufficient.

4. They meet the requirements specified by the Employerin the following scope:

a) offered deliveries

The Ordering Party will recognize that the condition is met if the offered subject of the order has been admitted to trading in Poland in accordance with the Act of 20.5.2010 on medical devices (Journal of Laws of 2015, item 876) and constitutes a medical device within the meaning of the above-mentioned Act

b) economic or financial ensuring order execution.

The Ordering Party will consider that-the condition is met if the Contractor proves that he has financial resources or creditworthiness for an amount not less than PLN 3, 000, 000.00 (in words: three million zlotys).

c) technical capacity guaranteeing the order execution

The Ordering Party will consider that the condition is met if The Contractor will show that in the last 3 years before the deadline for submission of tenders, and if the period of conductingia activity is shorter in this period, has performed or performs at least 3 orders, which included delivery, assembly / installation and commissioning of the computed tomography device,

and

the Employer considers that the condition is met if the Contractor shows that in the last 3 years before expiry of the deadline for submission of bids, and if the period of conducting business is shorter in this period, inz oo or performs at least 3 orders, which included robots consisting of adapting rooms for the needs of assembly and operation of a computed tomography device
