Delivery, installation and commissioning of 255 mobile ticket machines with software and guidance on the delivered sales service devices ticket tickets

The subject of the order is delivery, installation and commissioning of 255 itemsletomatow mobile with software and running ticket sales service on the devices provided. In addition, the Contractor's tasks will include delivery of three test sets. The subject of the orders will be executed in three stages. A detailed description of the subject of the order, terms of delivery, warranty, warranty and service has been specified in the draft contract constituting Annexe No. 6 to-SIWZ.

1. The subject of the orders includes:

1) Stage I:

a) delivery, assembly and commissioning of 255 vending machines in public transport vehicles with a frame and software and a central system (BackOffice), described in Annex No. 1 to the contract,

b) delivery of three sets test, described in Annex No. 1 to the contract.

2) Stage II:

Expansion and delivery of additional softwareTicket machines and the central system (BackOffice), described in Annex No. 1 to the contract, up to 265 units of dual function validators equipped with a self-service EMV payment terminal, purchased under another tendering procedure.

3) Stage III: - Provision of the service of selling tickets in public collective transport organized by the Białystok Commune, provided on the stage-And devices, in particular: electronic one-off, minute and short-bearer bearer tickets as well as top-up electronic purse on the Białystok City Card, as well as periodic tickets with a value that is acceptable for contactless payment cards transactions.

2. A detailed description of the subject of the order, delivery conditions, warranty, warranty and service has been specified in the draft contract, constituting Annex No. 6 to the ToR.

N / A - EXCLUSION OF EXCLUSION: 1. Contractors are excluded from the cases in cases specified in art. 24 sec. 1 Public Procurement Law Acts. 2. The Contracting Party provides for the exclusion of the Contractor based on art. 24 sec. 5 point 1 and items 5-8 of the Public Procurement Law

II. FOR A PRELIMINARY CONFIRMATION THAT THE CONTRACTOR NO-IS SUBJECT TO EXCLUSION AND MEETS CONDITIONS FOR PARTICIPATION IN THE PROCEEDINGS THE CONTRACTOR SUBMITTED:

1.Dolicy European Order Document / ESPD (hereinafter referred to as "EAT") on or according to Attachment 2 to the ToR. The document should be completed in accordance with the instructions, which can be found on the website. uzp.gov.pl, tab: Repozytorium wiedzy / Jednolity Europejski Dokumentacji Contracting. Pre-prepared formfrom the Employer for the subject procedure in electronic form (in .xml format - to be imported on the ESPD website) is available on the Ordering Party's website.

Note: In Part IV EAT (Qualification Criteria) The Contractor may only complete section a (general statement regarding all criteria qualification).

Statement of EAT, the Contractor should send to the Contracting Party afteran electronic station with a qualified electronic signature, in accordance with the rules set out in Chapter XVI items 3-5 SIWZ.

2.JSZ regarding other entities, if applicable.

3. Documents, in particular obligations of other entities to give the Contractor at the disposal of the necessary resources for the implementation of the order referred to in chapter XI point 2 point 2 of the ToR if applicable.

III.WYREPRESENTATIONS OR DOCUMENTS SUBMITTED BY THE CONTRACTOR IN THE PROCEEDINGS TO THE CALL FOR CONTRACTING AUTHORITY FOR THE CONFIRMATION OF CIRCUMSTANCES REFERRED TO IN ARTICLE 25 ITEM. 1 point 3 of the Public Procurement Law (concerning the highest rated offer):

1. The contractor submits the documents specified in Chapter. VIII point 1 items 1-9 SIWZ

2. Documents of foreign entities and contractors based in the territory of the Republic of Poland in relation to personsy residing in the territory of the Republic of Poland - determines the executive regulation to the Public Procurement Law

3. The Employer demands from the Contractor, which relies on the abilities or situation of other entities on the terms specified in art. 22a of the Public Procurement Law, present in relation to these entities documents in accordance with Chapter VIII point 4 of the SIWZ.

IV. Other documents: 1. In the case of Contractors applying jointlyo ordering, in order to confirm that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings:

1) EAT, referred to in Chapter VII point 1 of the ToR and declarations and documents referred to in Chapter VIII point 1 items 1-9 of the ToR shall be submitted by each of the Contractors,

2) documents referred to in Chapter IX SIWZ shall submit respectively the Contractor who demonstrates compliance with the condition, inand on the principles described in Chapter V point 1 item 3 of the SIWZ.

2. In the event that the Contractor, in order to confirm fulfillment of the conditions of participation in the proceedings, relies on the resources of other entities, the Contractor is obliged to submit:

1) EAT concerning other entities referred to in Chapter VII point 3 of the SIWZ in electronic form,

2) documents in writing together with the offer (in particular, the obligation-other entities to place the Contractor at the disposal of the necessary resources for the order fulfillment or according to Annex 4 to the ToR), which specify in particular:

a) scope of resources available to the Contractor of another entity,

b) manner of using the resources of another entity, by the Contractor, in the performance of the public order, - c) scope and period of participation of another entity in the performance of the contract

3. The Contractor, within 3 days from the date of publishing the information on the opening of offers on the website, provides the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law, along with other Contractors who submitted offers in this proceeding Annex No. 5 to the ToR.

V. Description of the method of prthe offer is being zigzagged and the documents included in the offer are indicated in ch. XVI SIWZ.
