The subject of this order is the supply of laboratory materials for the needs of the Medical University of Silesia in Katowice. The ordering party divided the subject of the order into 4 packages:

The subject of the order should be implemented based on the assumptionsspecified in the assortment and price specification constituting Annex 2.1.

1. Contractors who are not subject to exclusion pursuant to art. May apply for a public contract. 24 paragraph 1 point 12 23 Pzp

2. The Employer does not provide for the exclusion of the Contractor fromdepartment in these proceedings based on the conditions arising from art. 24 paragraph 5 Pzp.

3. The procedure is conducted in an open tender with application of art. 24 aa of the Public Procurement Law

4. Along with the offer must be submitted valid on the day of submission of offers statements on the EAT print. The information contained in JEDZ will constitute preliminary confirmation that the Contractor is not subject to exclusion from progressiveand meets the conditions for participation in the proceedings.

5. The Contractor, within 3 days of posting information from the opening of offers on the Employer`s platform available at https://sum.eb2b.com.pl/open-auctions.html, provides the Employer with a declaration of belonging or not belonging to the same capital group referred to in art. 24 paragraph 1 point 23.

6. Employer beforeby ordering the order, the Contractor whose tender has been rated the highest will be submitted within a specified period, not shorter than 10 days, valid on the day of submitting the following statements or documents:

1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids;

2) oproviding the Contractor with the lack of issuing a final court judgment against him or a final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the case of issuing such a judgment or a decision of documents confirming the payment of these debts together with any interest or fines or conclusion binding afterunderstanding on the repayment of these receivables;

3) the Contractor`s statement of the lack of a judgment against him as a preventive measure forbidding public procurement.

7. The proceedings are conducted in Polish in electronic form via the eB2B platform (hereinafter: the "platform") available at: https://sum.eb2b.com.pl/open-auctions.html. Contractor who ordersierza to participate in the public procurement procedure, must have an account on the platform. Registering and maintaining an account on the platform and using the platform is free. Technical and organizational requirements for using the platform are set out in the platform regulations (available at https://platforma.eb2b.com.pl/user/terms or https://tt.eb2b.com.pl/user/tERMS).

9. To send documents it is necessary to have a qualified certificate in order to sign the offer as well as statements and documents submitted in the proceedings. 10. Note: detailed information on the statements made by Contractors can be found in the Terms of Reference.

11. The Employer requires a deposit of:

- package No. 1 90 PLN,

- package No. 2 50 PLN,

- package No. 3 100 PLN,

- package No. 4 PLN 70.

12. Contractors with headquarters or placese residing outside the territory of the Republic of Poland. The issues of submitting documents by these Contractors are regulated by - 7 and - 8 of the Regulation on documents.
