Delivery of cameras and lenses

Reference Number: ZP / TITT / 113/2018

The subject of the order is the delivery of cameras and lenses.

Delivery of 18 camera tracks 4K. A detailed description of the subject of the order for individual parts of the order is contained in Annexes No. 1 to the ToR (at the same time Annex No. 1 to the contract template).

1. According to art. 8 sec. 2 a of the PZP Act, Zam. I inform thatall information that Zam. it has included a confidentiality clause and information closely related to them (eg clarification of the content of the ToR for confidential information).

2. In the case where in the proceedings Zam. will provide confidential information or information strictly related to confidential information, in order to preserve its confidentiality, in accordance with Art. 37 paragraph 6 of the PZP Act, they will not bee posted on the website Zam. Confidential information will be forwarded to the Contractor in one of the following ways chosen by the Employer:

a) personally at the headquarters of Zam. after confession by Zam. specific place, date and time. receipt,

b) by a messenger to the address wsk. via lect.

c) by e-mail

3. Offer and doc. and ośw. des. in the proceedings:

1) The offer on which Formularz of the offer, according to appendix 2a-2f to the ToR, for a given part of the order,

a) EAT (according to Annex 3 to the ToR), valid as of the date of submitting bids, in order to initially confirm that the lecture is not subject to exclusion from proceedings based on Art. 24 sec. 1 of the Act and meets the conditions for participation in progress. In order to apply for the order, EAT, each of the contractors applying for the order together,

b)EAT must be submitted electronically under pain of nullity in accordance with the requirements set out in point 9.3.6 of the ToR,

2) Ośw. or doc. submitted with the offer:

a) doc. Confirm. crops. to sign. offer and signature. or certify for from the original center of or doc. If separate regulations wym. entry into the register or, if applicable, doc. property is a copy from the property or from CEiDG or doc. on under. which entry is made-to the appropriate register or register, e.g. the resolution of the org. Lect.;

2) Ośw. about belonging or its absence to the same capital group, according to with the Act of 16.2.2007 on protection of concessions and consumers (Journal of Laws of 2018, item 798), referred to in art. 24 sec. 1 point 23 of the Act - subscribes to every Lecturer who submitted an offer - within 3 days from the day of on the website Zam. information (pursuant to Article 86 (5) of the Act).

4) If lecture is seated or place of order beyond ter Of the Republic of Poland, instead of the docs referred to in item 4, submit:

a) for in - 5 point 1 of - info. with resp. reg. or, in a footnote the absence of such a regime, another equivalent doc issued by the competent judicial body or admin. the country where Wy Wyk has its headquarters or place or place of order has a person who is concernedor info. or dok., in the scope of specify in art 24 sec. 1 points 13, 14 and 21 of the Act.

5) Requirements for the form of the submitted doc. were specified in the ToR.

6) Pursuant to art. 24 a a of the Act Zam. it will first evaluate the offers and then examine or perform the Call, whose offer has been assessed as the most advantageous, not subject to exclusion.

7) Reservation to the minimum period in which the Offer will be binding offersą, specify. in section IV.2.6. -Wyk. remains zw. submitted offer for a period of 60 days. The running of the term the offer begins with the deadline for submission of tenders.
