The Technical Assistance and Maintenance Service for managing the budget unit includes Program maintenance services and development services Program implemented as part of basic services and optional orders.

Main Sithese: Gdańsk

Technical Assistance and Program Maintenance Service, including Program maintenance services and Program development services, as well as the BI subsystem.

1) As part of the Program Maintenance service, the Contractor undertakes to: a) provide answers to Inquiries and assistance and advice in on using the Program in the form of telephone or written consultations for Administrators and Gothe Program's executors,

b) providing the service of removing any errors reported to the Contractor by the Employer,

c) assistance in the form of consultations in removing in the cooperation with the Program Administrator irregularities and disruptions in the operation of the Program resulting from the installation of hardware or external components and help for administrators in administering the Program,

d) performing the Update in the agreementfunctional scope of the Parties, e) providing the Ordering Party with subsequent versions of the Program or its Subsystems and related Documentation and providing the Employer with one copy of the installation along with a description on the IT data carrier, as well as installing the Program at the request of the Employer, Ordering Party's request for test results, i.e. T Scenariosest. to the Agreements, Updates, Modifications and Development Orders made under the Agreement,

g) making available or transferring and updating the Program Documentation within the terms and on the terms specified in the Agreement.

2) As part of the Program development services, the Contractor undertakes to implement the following services:

a) performing Program Modifications,

b) providing by the Contractoreń Rozwojowych,

c) execute, at the request of the Ordering Party, Program Maintenance services,

d) providing by the Contractor specified in the Agreement services on non-working days, not exceeding 12 days during the term of the Agreement. Each such day is equivalent to 12 Manhours,

e) providing the service of removing the Customer's Missteps reported to the Contractor by the Employer,

f) co-operation with the reconstructionThe first stage of the offer form should be accompanied by: - a statement from the Contractor (and another entity - if applicable), on the form JEDZ constituting preliminary confirmation:

a) fulfillment of the conditions for participation in the proceedings - b) not subject to exclusion from the proceedings for reasons referred to in art. 24 sec. 1 items 13-22 and 24 para. 5 points 1) Public Procurement Law - evidence intender bond - a commitment by another entity to place the necessary resources for the needs of the order - if applicable - a power of attorney to sign offers and declarations - if applicable - stage II

- within 3 days from the date of placing information on the Employer's website, which article art 86 par. 5 of the Public Procurement Law, the contractor is obliged to provide the Employerdeclaration of belonging or non-affiliation to the same capital group as other Contractors who submitted offers - stage III, upon the Employer's request: - - in the scope confirming no grounds for excluding

a) Information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Public Procurement Law Acts b) Declaration of the Contractor about the lack of a right to ita strong court ruling or final administrative decision on default of taxes, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming the payment of such claims together with any interest or fines or a binding agreement for repayment of such debts - c) Declaredand the Contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders

d) A copy from the relevant register or from central records and business information, if separate regulations require entry in the register or records, to confirm the absence of grounds for exclusion based on art. 24 sec. 5 point 1 of the Public Procurement Law Act

Purchaser requires Youa conqueror, who relies on the ability or situation of other entities, to present documents in respect of these entities, in the scope confirming the condition of participation by "other entity" and not subject to exclusion from participation - Documents submitted by foreign Contractors confirming no grounds for exclusion from participation in the proceedings - a) Contractor having pEither residence or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in the abovementioned point a) (Information from the National Criminal Record), submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence orthe residence of the person who is the subject of the information or document, to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the PPL Act;

b) If in the country in which the Contractor has its registered office or place of residence has the person concerned, the documents referred to in the abovementioned Act shall not be issued. point a) of this paragraph, shall be replaced by a document containing, respectively, providing the Contractor, indicating the person or persons authorized to represent it, or a statement of the person to whom the document was to be applied, submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the Contractor's seat or place of residence persons;

c) documents mentioned abovein point b-d stage III, at the request of the Awarding Entity in the scope confirming no grounds for exclusion.
