The subject of this contract is the provision of services related to the provision, maintenance and updating of the Veterinary and Food Information System Statistical Veterinary and Food Administration of the Slovak Republic through the following services:žieb:

Providing services related to the provision, maintenance and updating of the Veterinary and Food Information System of the State Veterinary and Food Administration of the Slovak Republic according to the current requirements.

1.1 The public procurement may be awarded only to those who meet the following conditions:the part concerning the personal status according to - 32 par. 1 of the Public Procurement Act, which proves:

a) a letter. a) a documented extract from the criminal record not older than 3 months, b) the file. (b) proof of health insurance and social security attestation not more than 3 months; c) attested by the local tax authority not later than 3 months; d) documented pby the seizure of the relevant barrel not more than 3 months; (e) a document proving the right to deliver the goods, to perform construction work or to provide a service which corresponds to the subject of the contract; f) documented declaration of honor

1.2 Agents entered in the list of economic subjects pursuant to - 152 of the Public Procurement Act, which the Public Procurement Office can proveof the conditions of the person's personal status under Section 32 1 letter. (a) to (f) and 2, 4 and 5 of the Act on Public Procurement by submitting a valid certificate of the office for its inclusion in the list of economic entities.

1.3 A group of suppliers proves that the conditions for the personal status of each individual member of the Group are fulfilled. The service can only be corrected by a member of the group only inin relation to that part of the subject matter of the order which I shall provide.

1.4. 4 of the Act on Public Procurement, the applicant may be interested in the site, place of business or habitual residence outside the territory of the Slovak Republic from the documents pursuant to Section 32 2 of the Act on Public Procurement to be replaced by an affidavit if the status of his or her place of business, place of business or usualthe requested document, or the equivalent document does not issue.

1.5 Pursuant to the provisions of - 32 par. 5 of the Act on Public Procurement Applicant or occupier with a seat, place of business or habitual residence outside the territory of the Slovak Republic, unless the right of this statute is regulated by the Institute of Honorary Denunciation, may be replaced by an affidavit made by a court, a body, a notary or a municipality• The documents must be original or certified by them, not older than 3 months as of the date of submission of the tender. the contracting authority has determined the contracting authority in accordance with the provisions of - 32 of the public procurement law

Proof of part-time conditions in accordance with Section 39 of the Public Procurement Act Single European Document (JED):

1. A single European document on the application of the Public Procurement Act is a document which the economic entity may provisionally replace the documents proving the fulfillment of the conditions of the individual's personal status, financial and economic status and technical supportor professional competence, designated by the contracting authority. The Single European Document contains an updated declaration of the economic entity that: a) there is no evidence of its exclusion, b) it provides the contracting authority on request with the documents which he has replaced with a single European document. If an applicant or occupier demonstrates the financial and economic standing or technique of the assistantability or professional competence through another person, a single European document shall also contain information about that person. If the tender is submitted by a group of suppliers, a single European document is submitted by each member of the group of suppliers. The economic operator shall indicate in a single European document other relevant information as required by the contracting authority, the bodies and entitiesvaju documents proving the fulfillment of conditions of part time. If the required documents for the contracting authority are accessed directly and free of charge in electronic databases, the economic entity in the single European document shall also provide the information necessary for access to those electronic databases, including the Internet address of the electronic databases, any identification data andinto this database. These documents shall not be required by the contracting authority and the required information shall be obtained on the basis of access to the electronic databases. If the applicant or occupier uses a single European document, the contracting authority may ask for a document or d) to ensure the proper conduct of the procurement at any time in the course of the tenderer or occupierreplaced by a single European document. The assignor or receiver delivers the documents to the contracting authority within five working days of the date of receipt of the request if the contracting authority or the contracting authority has not specified a longer period.
