Supply of CO laser2 with a micromanipulator, laryngological drill with a control console, ENT set for electrosurgery with radio frequency (RF) current, ENT chair, CO2 insufflator, medical scales, patient transport carriage in a sitting position, holmium laser with dedicated mini PCNL kit and uretrotome, autoclave cassette, three-plane ultrasound headand a headlamp, broken down into 11 parts, described in detail in attachments No. 4.01-4.11 to siwz.

The Ordering Party does not set conditions in the above-mentioned in scope.

Documents submitted at the request of the Employer - statements and documents regarding the demonstration of the lack of grounds for exclusion of the contract. from participation in the progress ofbacked up in ch. VIB section 2 siwz:

1.Information from the NQF in the scope specify. in art 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law, exhibitions. no earlier than 6 months before the expiry of the term composition. offers.

2. The certificate of the competent head of the US confirms that the contractor. he is not in arrears with paying taxes, issued not earlier than 3 months before the expiry of the term composition. offers, or other documentary evidence that youKonawa. entered into an agreement with the competent tax authority in repayment of these receivables with possible interest or fines, in particular obtained legally required exemption, postponement or spreading into installments of overdue payments or suspension in full of the implementation of the decision of the competent authority.

3. Certified. the relevant local organizational unit of ZUS or KRUS or other document pconfirming that it is performed. he is not in arrears with paying social security contributions. or health. issued not earlier than 3 months before the expiry of the term composition. offers, or other documents, confirm that the contractor has concluded an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular obtained the exemption provided for by law, postponementor spreading overdue payments or stopping in full the decision of the competent authority.

Description of the relevant register or central records and information on economic activities, if separate provisions require entry in the register or records, to confirm. no grounds exclude. on the basis art.24ust. 5 point 1 of the Act.

5. Certify. implementing. about not publishing it to himcourt decision or a final administrative decision on default of taxes, fees or insurance premiums. social. or health. or in the event of such a verdict or decision being made. confirm. making the payment of these receivables with any interest or fines or entering into a binding agreement in repayment of these receivables, drawn up in accordance with Annex nr 5 to siwz (original).

6. Declaration of the contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders and a declaration of non-payment of taxes and local fees, referred to in the Act of January 12, 1991. on taxes and local fees, prepared in accordance with Annex No. 5 to SIWZ (original document). 7. If itin. has a registered office or place of residence outside the territory of the Republic of Poland, instead of the documents:

7.1 referred to in Chapter VI B para. 2.1. Siwz shall submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative authority of the country in which it is performed. has a registered office or place of residence or place of residence has a person who is involved in the information. or a document, in the scope referred to in Article 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law - presented no earlier than 6 months before the deadline for submission of tenders;

7.2 referred to in Chapter VI B para. 2.2 2.4 siwz shall submit a document or documents issued in the country in which it is performed. has a registered office or place of residence, confirming that:

7.2.1 is not in default of paying taxes, fees, contributions-for insurance. Socio. or health. or that he has entered into an agreement with the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular he obtained the legally required exemption, deferral or installment payment of overdue payments or suspension in full of the decision of the competent authority of the exhibitions. not earlier than 3 months before the deadline,

7.2.2its liquidation was not opened or bankruptcy announced - issued not earlier than 6 months before the deadline for submitting bids.

The Ordering Party does not set conditions in the above-mentioned extent.

2. The Contractor may, in order to confirm fulfillment of the conditions for participation in the proceedings, referred to in Chapter V, para. 1.2.3 siwz, in the situations and in relation to a specific order, or its part, consistson the technical or professional abilities of other entities, regardless of the legal nature of the legal relationships that it has with it.

2.1. The Employer at the same time informs that the "appropriate situation" referred to above will occur only when:

2.1.1 The Contractor, who relies on the abilities of other entities, will prove to the Ordering Party that, by implementing the order, he will have the necessaryresources of these entities, in particular by presenting the obligation of these entities to provide them with the necessary resources to fulfill the order;

2.1.2 The Employer will assess whether or not the technical or professional capacity provided by other entities can be demonstrated to the Contractor to satisfy the exclusionary basis, referred to in art. 24 sec. 1 item 13-22 and para. 5-point. 1, 2, 4 and 8 of the Act on Public Procurement Law;

2.1.3. If the technical or professional capacity of the entity referred to in art. 22a paragraph 1 of the Public Procurement Law, do not confirm the fulfillment by the contractor of the conditions of participation in the proceedings or are subject to the exclusion grounds for these entities, the contracting party requests that the contractor, within the time specified by the contracting authority: - 2.1.3.1 replaced the entity with another entitytem or entity or - 2.1.3.2 has committed to the personal implementation of the relevant part of the contract, if it shows technical or professional capacity referred to in art. 22a paragraph 1 of the Public Procurement Law.

2.2. The obligation or other documents confirming the availability of resources by other entities must clearly and unambiguously result in particular:

2.2.1 scope availablecontractors of resources of another entity,

2.2.2 manner of using the resources of another entity, by the contractor, while executing the order,

2.2.3 scope and period of participation of another entity when performing the public order;

1. Statement. and documents submitted by the executor along with the offer: 1.1. The declaration on the single European order document form is valid as of the day of submitting bidsia (EAT), according to Annex No. 3 to SIWZ (original document).

CAUTION:

The Purchaser informs that when completing Part IV of the EED form under the name "Eligibility Criteria", the Contractor may confine himself to filling in section a, i.e. to submit general declaration regarding all qualification criteria, in which case the Contractor does not have to fill in any of the other sections of the partand IV of the qualification criteria, and the proper (evidence-based) verification of compliance with specific conditions of participation in the procedure specified by the Employer (selection criteria), the Employer will make based on the relevant documents specified in Chapter. VI part B para. 1 SIWZ.

1.2. In the event of a joint application by contractors, a uniform European document will be ordered(EAT) is submitted by each of the contractors jointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion to the extent that each of the contractors demonstrates fulfillment of the conditions for participation in the proceedings and no grounds for exclusion, in accordance with Annex 3 to SIWZ (original document).

1.3. Contractor who refers to the resourcey other entities, in order to demonstrate the absence of grounds for exclusion and fulfillment to them, to the extent that they refer to their resources, the conditions for participation in the proceedings also include single European order documents (EIN) regarding these entities, in accordance with Annex 3 to siwz.

1.4. Documents indicated in Chapter VIA (4 and 5 siwz- in the case, when the contractor relies onbones of other entities

Declaration. and a document. folded in term 3 days from the day of publishing. on the internet page. information about which we speak. 86 par. 5 Public Procurement Law:

Statement inthe contractor for membership or non-affiliation to the same capital group, the contractor submits within 3 days from the date of posting on the website the information referred to in Article 86 par. 5 of the Public Procurement Law, in accordance with Annex No. 6 to SIWZ (original document).

In the case of belonging to the same capital group, the contractor may submit together with the declaration documents or informationconfirming that connections with another contractor do not lead to distortion of competition in the proceedings - in the case when the contractor does not belong to any capital group within the meaning of the Act of February 16, 2007 on competition and consumer protection (Journal of Laws of 2017, item 229, as amended), the orderer allows submission of the aforementioned statement together with the offer.

In the case referred to in the sentencewhen, from the date of signing the declaration to the date of placing by the customer on the website of the information referred to in Article 86 par. 5 of the Public Procurement Law, any change in the contractor's situation will take place, i.e. inclusion in the capital group, the contractor is obliged to promptly update the submitted statement within 3 days from the date of posting on the website infthe ormation referred to in Article 86 par. 5 of the Public Procurement Law Act
