Subscription to domestic and foreign press in print and electronic form

The subject of the order is subscription to domestic and foreign press in print and electronic form. The contract is awarded in four parts, including:

Main Site: Locations in the Capital City of Warsaw Warsaw, i.e.: al. J. Ch. Szucha 21, al. J. Ch. Szucha 23, ul. Taneczna 67, ul. Krywulta 2, ul. Karmazynowa 1A, ul. Miodowa 10, POLAND.

1. The subject of the order is the systematic delivery by the Contractor of a national printed press for the Ministry of Foreign Affairs (as per the list in item 19 OPZ).

2. The press will be delivered to the Employer`s facilities located in the Capital City of Warsaw. Warsaw, i.e.: al. J.Ch. Szucha 21, al. J.Ch. Szucha 23, ul. dance67, street Krywulta 2, ul. Karmazynowa 1A, ul. Miodowa 10.

3. In the event of changes to the premises of the Employer, referred to above, the place of delivery may change during the performance of the contract, however, they will always be located in the Capital City of Warsaw Warsaw. Details regarding the change of delivery place will be provided to the Contractor on an ongoing basis in a working mode by electronic means. Contractor`s obligationsy is to introduce a change within 2 business days of receipt of the notification of a change of place of delivery.

4. The printed press will contain all the extras included in the price of the given title.

5. The deliveries that are the subject of this contract will start from May 8, 2020 and will be carried out for the next 12 months or until the earlier use of the Contractor`s remunerationof the contract.

A detailed description of the subject of the contract is attached as Annex 1a to the ToR.

The Employer does not set conditions in this respect. Contractors who can apply for the contract who:

a) are not excluded from the procedure pursuant to art. 24 paragraph 1 point 12 23 of the Act;

b) meet the conditions for participation in the proceedings regarding the capacity ofechnical or professional.

2. In order to initially confirm that the conditions for participation in the procedure specified in the ToR are to be met and that there are no grounds for exclusion, the Contractors are required to submit, together with the offer, a current statement as at the date of submitting the bids in the scope indicated by the Employer in the ToR, in the form of a single document (EAT).

3. The contractor within 3 days from the date of publicationon the Employer`s website, the information referred to in art. 86 section 5 of the Act (information from the opening of offers), is obliged to provide the Employer with a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 item 23 of the Act, with Contractors who have submitted separate bids in the said procedure.

4. Ordering in front of the thighI will call the Contractor whose offer has been highest rated to be submitted within a specified period, not shorter than 10 days, valid as at the date of submission of the following statements or documents:

a) list of supplies / services performed, and in the case of periodic or continuous services performed within the last 3 years before the offer submission deadline, and if the period of workof operations is shorter in this period, together with their value, subject, dates of performance and entities for whom the services have been rendered, and attachments of evidence specifying whether these supplies / services have been performed or are being performed properly. The template of the list is, respectively, Annex 7a d to the ToR;

b) information from the National Criminal Record in the scope specified in art.24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids.

The above information should be submitted in the scope of: the Contractor being a natural person or a collective entity and the members of the management or supervisory body, the company`s shareholders in a public company or partner or general partners in a limited partnership or limited joint-stock partnershipc) a statement by the Contractor that he has not issued a final court judgment or final administrative decision against him in defaulting in payment of taxes, fees or social security or health insurance contributions, or in the case of such judgment or decision, documents confirming the payment of these amounts together with any interest or fines, orb conclusion of a binding agreement regarding the repayment of these claims. A specimen statement is attached as Annex 8 to the ToR;

d) the Contractor`s statement that he has not been subjected to a preventive measure against him from applying for public contracts. A specimen declaration is attached as Annex 8 to the ToR.

5. Documents made in a foreign language are submitted together with a translation into Polish.

6. remainede requirements for declarations and documents submitted by the Contractors are contained in the order documents (SIWZ).

7. The contracting authority may, in accordance with art. 24aa section 1 of the Act, first evaluate the offers, and then examine whether the Contractor whose offer has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure.

8. The Employer requires bringing defectsium in the amount, respectively, for individual parts of the order:

a) Part I of the order: PLN 2, 000.00 (in words: two thousand, 00/100);

b) Part II of the order: PLN 2, 000.00 (in words: two thousand, 00/100);

c) part III of the order: PLN 7, 000.00 (in words: seven thousand, 00/100);

d) part IV of the order: PLN 1, 900.00 (in words: one thousand nine hundred zlotys, 00 /100).
