2. Subject of the orderand there is a supply of brand new, unused scientific and research apparatus to the Institute of Archeology of the University of Wrocław together with assembly and installation - 3 parts:
Part 1 Table X-ray fluorescence spectrometer for microanalysis
2.1. A detailed description of the subject of the contract is included in Annex 3a to the ToR. Description of the subject of the contract respectively for: Part 1.
2.2. Detailed terms and conditions of order processingof payment and payment is specified in the Contract template, constituting Annex 4 to the ToR.
1. Contractors who are not subject to exclusion from the contract award procedure pursuant to art. 24 paragraph 1 and art. 24 paragraph 5 point 1 of the Public Procurement Law;
2. Activities carried out for the purposes of performing the contract object do not require special permissions.
3. Everyone in the offerthe contractor must attach a current statement as at the date of submitting the bids in the scope indicated in Annex 2 to the ToR submitted in accordance with the provisions of Chapter VII point 2 point 4 of the ToR (EAT).
4. Documents submitted at the request of the contracting authority:
1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the expiry of the terminu to submit bids;
2) an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or register, in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Public Procurement Law;
3) the contractor's statement that he has not issued a final court judgment or final administrative decision regarding himdeclining the payment of taxes, fees or social security or health insurance contributions, or in the event of issuing such a judgment or a decision of documents confirming the payment of such receivables together with any interest or fines or concluding a binding agreement on the repayment of these receivables - Annex 7 to the ToR;
4) declarations of the contractor about the lack of judgment against nas a preventive measure of the prohibition to apply for public procurement - Annex 8 to the ToR;
5) documents regarding third parties, in order to demonstrate the absence of grounds for exclusion for them if the contractor relies on the resources of a third party;
6) in the case of an offer by contractors jointly applying for the procurement of documents regarding eacho from contractors to demonstrate that there are no grounds for exclusion.
If the Contractor indicates in the Form an offer, generally available, free electronic databases, the Employer will use these documents to confirm the circumstances referred to in art. 25 paragraph 1 point 3 by downloading them yourself.
If the statements and documents referred to above are preparedare in a foreign language, the Contractor is obliged to submit their translation into Polish. 5. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, he shall submit documents in accordance with the provisions set out in the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in a proceeding for a grantno orders, and indicated in the ToR in chapter. VI points 8, 9 and 10.
The Purchaser does not set a detailed condition in this respect.
1. The offer in its content must correspond to the content of the Specification of the essential terms of the order. The offer, statements and documents for which the Employer has specified templates of attachments to this ToR should be made in accordance with these templates as to the content and description ofcolumns and poems.
2. For the purposes of examining and evaluating offers, the offer must contain the following statements and documents:
1) Offer form - Annex 1 to the ToR - in the original in electronic form with a qualified electronic signature. for a given part) in points Ia, Ib, Ic, in row E (Part 1, respectively)2, 3) the manufacturer and model of the equipment offered. This information is the content of the offer, therefore, if the Contractor fails to provide this information, his offer will be rejected pursuant to art. 89 paragraph 1 point 2 as not corresponding to the content of the ToR (Part 1, 2, 3, respectively);
2) current statements on the lack of grounds for exclusion and meeting the conditions for participation in the procedure in accordance with art. 25a of the PPL referred to in chapter VI point 1/5 - in the original in electronic form with a qualified electronic signature;
3) power of attorney for the person / persons signing the offer to make commitments on behalf of the contractor submitting the offer, if the right to sign the offer is not apparent from other documents attached to it in the original in electronic form, qualifiedelectronic signature of the person / persons authorized / authorized to grant the power of attorney;
4) in the case of submitting a joint bid by several Contractors, the power of attorney should be presented in the original in electronic form with a qualified electronic signature of the person / persons authorized / authorized to grant the power of attorney, establishing the proxy for representContractors in the procurement procedure or representing and concluding the public procurement contract. The content of the power of attorney should indicate the type of rights to which the proxy is authorized;
5) the obligation of another entity (third parties) to provide the Contractor with the necessary resources at the disposal of the contract (if applicable) Annex 9 to SIGM in the original in the form of an electronic document or electronic copy of the document or statement certified as true copies.
2. The Awarding Entity does not provide for the possibility of awarding contracts referred to in art. 67 section 1 point 7 of the PPL Act
3. The Contractor entering the tender is required to submit a deposit in the amount of: Lot 1: PLN 5, 000 (say: PLN: five thousand); Hey2: PLN 10, 000 (in words: PLN ten thousand); Lot 3: PLN 10, 000 (in words: PLN ten thousand).
4. The Contractor within 3 days from the date of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, will provide the contracting authority with a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act (according to the model - Annex 5 to the ToR). The Contractor is obliged to provide this statement in the original in the form of an electronic document or an electronic copy of the document certified as being true to the original - in accordance with Chapter VII point 3 point 2. Together with the submission of the declaration, the contractor may provide evidence that links with another contractor do not lead to distortion of competition inProceedings for the award of the contract. In the case of joint application for the contract by the Contractors, a declaration is made by each of the Contractors jointly applying for the order.
5. The Contractor submits the offer via the Form to submit, amend, withdraw the offer or application available on ePUAP and also made available on the miniPortal. Public key necessary to encrypt the offeryou through the Contractor is available to contractors on the miniPortal or in Annex 11 to the ToR.
