Purchase and delivery of atomic absorption spectrometer and MAS LC-MS / MS spectrometer set. Reference Number: AZAPUZA / 40/19 / PN

The subject of the order is: purchase, delivery, assembly and commissioning in the registered office of the Ordering Party, in the place indicated by the Ordering Party, i.e. the Institute of Occupational Medicine prof. Dr. med. Jerzy Nofer in Ł-dź at ul. St. terfrom the Infant Jesus No. 8, POLAND, a brand new atomic absorption spectrometer (part 1) and an LC-MS / MS mass spectrometer set (part 2).

Part 1 (detailed description can be found in Annex 1A)

Device:

Spectrometer atomic absorption, adapted to analysis in a graphite furnace with the possibility of connecting the system to the generation of hydride / cold mercury vapor, completely controlled byinternal PC.

- Littrov monochromator with automatic wavelength setting working in the range not exceeding 190-900 nm,

- optical fiber light transmission system for analyzing radiation inside the spectrometer,

- diffraction grating with parameters not worse than: 1800 lines / mm optimized for 236 nm and 597 nm,

- at least an 8-position lamp changer coded from the machinewavelength setting, gap, lamp current and lamp position relative to the optical path,

- built-in minimum 2 lamp power supply with increased light efficiency.

4. The Contractor is obliged to train the users in particular in the scope of service, operation, software and providing substantive help.

5. The Contractor is obliged to provide a guarantee for the device for the period of minimum 12 months (part 1) and min. 12 months (part 2) and warranty for other devices being the subject of the order in accordance with the date indicated in the manufacturer`s guarantee documents. The Contractor will take longer warranty periods for subassemblies, if they have one at their producers. The guarantee runs from the date of assembly, ie the signature of the transfer protocol to the exploatation (delivery-acceptance protocol).

6. The Contractor is obliged to provide the Employer with warranty and post-warranty service provided by the entity having the manufacturer`s authorization in Poland and the ability to remotely monitor the condition of the device under warranty or service contract by the service department

7. The Contractor is obliged to provide the Employer with the availabilityspare parts for the device being the subject of the order for the period: - - minimum 7 years (part 1)

- minimum 8 years (part 2).

8. The Contractor undertakes to install and put into service the object of the order at the headquarters of the Ordering Party and at the place indicated by him and undertakes to provide free telephone assistance from 9:00 to 17:00 / working days (ie Monday- Friday) applies to the subject matter of the orders part 1 and 2;

9. Within the scope of the guarantee, the Contractor is responsible for the quantity and quality deficiencies revealed immediately after unpacking the subject of the contract at the Employer

10. The subject of the orders (parts 1 and 2) will be delivered to the registered office of the Ordering Party, to the place indicated by the Ordering Party (the service also applies to bringing the equipment) by transportat his expense and risk in a way that protects him from damage. The contractor will assemble and install the equipment at the headquarters of the Ordering Party, commission the device and carry out tests to verify the correctness of operation. The commissioning of the subject of the order (parts 1 and 2) will take place under the supervision of the persons indicated by the Ordering Party.

11. The Contractor undertakes to make the Purchasergo without any additional charges:

- commissioning the subject of the order (parts 1 and 2);

- conduct training of the Employer`s personnel in Polish in the field of handling ordered equipment, their software:

Part 1 - training 3 days 1 day installation training

Part 2 - training 3 days 1 day installation training

12. The Contractor shall provide the Ordering Party with the subject of the contractenia (parts 1 and 2) operating instructions in Polish and English, stamped guarantees.

The ordering party withdraws from the condition in this respect

The ordering party waives the condition in this respect

I. When choosing the offer, the Employer will be guided by the following criteria, which have been assigned the meaning:

Part 1:

1. Price (C) - 60%

2. Guarantee 40%

The way to calculate the number of punktow in the criterion of guarantee.

In this criterion, the Contractor can obtain a maximum of 40 points

Part 2:

1. Price (C) - 60%

2. Technical parameters: 40%

II.1. The Contracting Party requests that the Contractors pay a bid bond.

2. The Bidder`s bid bond must be submitted before the deadline for submission of offers indicated in this ToR.

3. The bid bond should be paid in the amount of:

a) part 1 - PLN 8 240.00 (eight thousand two hundred and fourzlotys 00/100)

b) part 2 51 360, 00 PLN (fifty one thousand three hundred sixty zlotys 00/100)

4. The bid bond can be contributed in one or several of the following forms: - a) money, - b) bank guarantees or social and credit union guarantees, except that the surety is always a cash guarantee;

c) bank guarantees;

d) ube warranty(e) sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2014, item 1804 and 2015, item 978 and 1240).

III. 1. The Employer demands from the Contractor security on due performance of the contract in the amount of 2% (in words: two percent) of the total price specified in the offer.
