The subject of the order is the provision of sanitary and medical transport services understood as transport of patients (life-threatening transport, consultations and diagnostic tests, transferring the patient to another medical entity, transport to the homeu) and other services resulting from the Employer`s activities 24 hours a day / 7 days a week / for a period of 12 months.

Pursuant to art. 29 paragraph 3a of the Public Procurement Law, the Employer requires employment by the Contractor or a subcontractor on the basis of an employment contract of persons performing activities indicated by the Employer in the implementation of the contract, in the manner specified in art. 22-1 of the Act of June 26, 1974, Labor Code. The contracting authority requires that persons employed directly atthe performance of activities related to driving vehicles, drivers were employed on the basis of an employment contract (not applicable to doctors, nurses, paramedics, the requirement applies to tasks: 1, 2 and 3.

In accordance with Article 13 (1) and (2) of the Regulation of the European Parliament and Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of dataand on the free movement of such data and repealing Directive 95/46 / EC (general regulation on data protection) (Official Journal EU L 119 of 4.5.2016, p. 1), hereinafter "GDPR", I inform you that:

- the administrator of your personal data is the Independent Public Health Care Center of the Ministry of the Interior and Administration with the Warmian-Masurian Oncology Center based in Olszat Al. Wojska Polskiego 37;

- your personal data will be processed on the basis of art. . 6 clause 1 lit. c GDPR for the purpose related to the procedure for the award of a public contract entitled: "Medical and sanitary transport service", case mark: ZPZ-11/03/20, conducted in an open tender;

- recipients of your personal data will be persons or entities to whom the documentation of proceedings based on art. 8 and art. 96 section 3 of the Act of 29 January 2004 Public Procurement Law (Journal of Laws of 2017, item 1579 and 2018), hereinafter "PPL Act";

- your personal data will be stored, in accordance with Article 97paragraph. 1 of the Public Procurement Law, for a period of 5 years from the date of termination of the contract award procedure. the consequences of not providing specific data result from the Act of the Public Procurement Law

With reference to Pnor your personal data decisions will not be taken in an automated manner, applying to art. 22 GDPR.

You have:

- based on art. 15 GDPR, the right to access your personal data concerning you,

based on art. 16 GDPR, the right to rectify your personal data **,

- pursuant to art. 18 GDPR, the right to request the administrator to limit the processing of personal data fromcase law, referred to in art. 18 clause 2 GDPR ***,

- the right to lodge a complaint to the President of the Office for Personal Data Protection, if you feel that the processing of your personal data violates the provisions of the GDPR.

You are not entitled:

in connection with art. . 17 clause 3 lit. b, d or e GDPR, the right to delete personal data,

- the right to transfer personal data, about whichmentioned in art. 20 GDPR,

- pursuant to art. 21 GDPR, the right to object to the processing of personal data, because the legal basis for the processing of your personal data is Art. 6 clause 1 lit. c GDPR.

The Employer does not define a special condition for participation in the proceedings in this regard. To initially confirm that the conditions for participation in the procedure have been met, the Awarding Entity requires submission in uniformm the European document ordering the statement only in Part IV, section α.

This condition will be met if the Contractor has a document confirming the possession of a currently paid civil liability insurance policy in the scope of conducted business activity and liability for damage resulting from non-performance or improper performance of the contract, about totalwarranty in the amount of at least PLN 300, 000.00. To initially confirm that the conditions for participation in the procedure are met, the Employer requires that the declaration in the Single European Order Document be submitted only in Part IV, section α.

I. The Contractor joining the procedure is obliged to pay:

Task 1 4 510.00 PLN

Task 2 1 276.00 PLN

Task 3 7 258.00 PLN

II. The Employer provides for the use of so-called "reverse" procedure in accordance with Article 24aa of the Public Procurement Law

III. The Employer does not provide for supplementary orders referred to in Article 67 paragraph 1 item 6 and 7 of the Public Procurement Law and Article 134 paragraph 6 item 3

IV. The contracting authority informs that it does not intend to conclude a framework agreement, it does not intend to establish a dynamic purchasing system, it does not envisage the use of an electrical auctioncations.

V. The Employer will exclude Contractors from the proceedings on the basis of the conditions arising from art. 24 paragraph 1 point 12 23 and from art. 24 paragraph 5 points 1 and 8 of the Public Procurement Law

VI. In order to demonstrate the lack of grounds for exclusion, the Employer requests Contractors to submit EED (preliminary statements) and documents referred to in - 5 point 1 6 and 9 and 10 of the Regulation of the Minister of Development of 26 July 2016 onalmost the types of documents that the Employer may request from the Contractor in the procurement procedure.

VII. In order to demonstrate compliance with the conditions for participation in the procedure, the Awarding Entity requires Contractors to submit EED (initial statements) and a current, paid civil liability insurance policy in the scope of conducted business activity and liability for damageę resulting from the non-performance or improper performance of the contract, with a guarantee sum of at least PLN 300, 000, a list of vehicles sent to perform this order in accordance with the attachment to the ToR, a statement about having sufficient staff with the appropriate qualifications necessary to properly complete the subject of the contract in accordance with the attachment to ToR, list inminimum 2 services rendered in the last three years before the offer submission deadline, and if the business period is shorter in that period, corresponding to the type and value of services being the subject of the contract, i.e. consisting in the provision of sanitary and medical transport services understood as transport patients with a value of not less than 100, 000.00PLN, stating the value, subject, dates of performance and entities for which the services have been rendered, and attaching a confirmation confirming that these services have been properly performed. Certification in the case of services still being performed should be issued not earlier than 3 months before the deadline for submitting bids in accordance with the attachment to the ToR.

VIII. Performers based outside the gamein the place of the documents of the Republic of Poland in place of the documents specified in - 5 item 1 6 and 9 of the Regulation of the Minister of Development of 26 July 2016 regarding the types of documents that the Employer may request from the Contractor in the procurement procedure, submit the documents specified in - 7 and 8 of this Regulation .

IX. Detailed information on the above-mentioned documents and other documents required inproceedings are contained in the Specification of the Essential Terms of the Order.

X. The Employer informs that he has an account for electronic invoicing on PEFexpert, an electronic invoicing platform enabling the Contractor to submit an electronic invoice.
